• Persons hired for this position are responsible for ensuring that competent, safe, quality, and professional care is provided to each of the patients assigned to his/her caseload or other team members (in their absence, in accordance with current applicable Federal, State, and local standards, guidelines and regulations.
• Utilize knowledge of chemical dependency and counseling techniques to introduce effective education and counseling for patients under their care.
• Persons hired will conduct individual and group counseling to meet treatment plan goals and objectives.
• Produce a clear and concise documentation in all areas including, but not limited to: Admit note, assessments, daily charting, weekly clinical review, staffing notes, treatment plan updates, biopsychosocial, transition and discharge planning, documentation of family sessions as needed and all summaries.
• Responsible for appropriate referral to each patient in need, open clear and ethical communication with referral sources; follow up of each of his/her cases after discharge.
• Motivates patients for changes, effectively performs crisis interventions, screening, intake, orientation, assessment, treatment planning, counseling (group and individual, and family).
• Appropriately and professionally provides case management, patient education and keep clear, ethical, and confidential reports and record keeping.
• Interacts and relates with peers and other professionals to assure comprehensive and quality care for the patients.
• Will participate in Crisis Counselor after-hours phone line rotation.
