Western CT Area Agency on Aging is Hiring a Resident Services Coordinator Near Naugatuck, CT
General statement of duties:
The WCAAA seeks a part time (14 hrs/wk) Resident Services Coordinator (RSC) who will work in a residential, service setting assisting disabled/elderly residents. The RSC will maintain regular contact with residents, assess their individual needs and work with them to establish or maintain linkages to needed supportive services, desired or required to maintain independence. RSC will also create a monthly newsletter, provide case management, advocacy, and information and referral, related to supportive and social services for eligible residents; provide up-to-date information and clarification regarding programs such as Medicare, Medicaid, entitlements, formal supportive and social services. Develop care plans in coordination with assessment services in the community, and the Professional Assessment Committee (PAC), where applicable.
Qualifications:
Candidate should hold a bachelor’s degree in social work, Psychology, Human Services, or related specialty OR demonstrated 2 years’ work experience relevant to the position. Should possess knowledge of the aging network, intervention techniques, and housing policies. Successful candidate must be able to lift & carry at least 10lbs (example: laptop). Must have own vehicle & valid driver’s license. Work site in Naugatuck, CT.
