Summary
We are seeking to hire a Case Manager to assist the senior and adult disabled population in maintaining themselves in their homes as long as possible. Develop, implement, and coordinate the plan of care.

This position is 37.5 hours per week with a pay rate of $22/hour. Westbay offers a generous benefits package including 3 weeks paid vacation accrued per year, 10 sick days accrued per year, 2 personal days per year as well as 14 paid holidays plus a paid day off on your birthday. Other benefits include life, health, dental, and vision insurance, and a 401k with company match.
