Job Description Summary:
The Care Coordinator provides targeted support services, via the wraparound process, to families with high risk young children/children/youth/transition age youth, regarding behavioral or emotional problems, parenting issues, and life stressors by developing and implementing bio-psycho-social assessments and service plans with the family and wraparound team members. The Care Coordinator utilizing the System of Care (SOC) philosophy will work with families, social service organizations, and other state agencies to develop a coordinated network of resources and supports in their communities.
JOB SUMMARY/SCOPE OF RESPONSIBILITY:
Under the supervision of the Clinical Manager, the PATH Coordinator - Project Rural Recovery (PRR Coordinator) is responsible for providing, coordinating, and monitoring the delivery of services regarding the driving and maintenance of the mobile unit and the service delivery coordination for Project Rural Recovery. Responsibilities of PRR Coordinator require driving and maintaining the Mobile Unit, facilitating relationships and scheduling clinic days with community partners, maintaining the Unit’s schedule, and facilitating smooth operation of clinic during clinic days. The PRR Coordinator generally works eight (8) hours per day, five (5) days per week and is available if needed for consultation at other times regarding the mobile unit and the schedule.
ESSENTIAL JOB FUNCTIONS:
Provides client and family education, support, and care in gender-, spiritually-, economically- and culturally-responsive, humble, kind, and competent ways Follows or exceeds HIPAA guidelines and maintains the highest level of confidentiality and privacy at all times when dealing with client information Provides services to people in recovery to promote individual, program and system-level approaches that foster health and resilience, increase permanent housing, employment, and other necessary supports while reducing gaps in service and barriers to social inclusion Drives, operates, and maintains the Mobile Unit in concert with West Tennessee Healthcare (WTH) Fleet services Ensures the unit meets standards and requirements at all times with WTH, Tennessee Department of Transportation (TDOT), and Tennessee Department of Mental Health & Substance Abuse Services (TDMHSAS) Works to build and maintain relationships with partner agencies in the service area Develops and maintains a clinic schedule for the unit that ensures time is given to meet the needs of each partner location and county Coordinates schedules of Provider, Case Manager, and Peer Support Specialist for staffing, community engagement events, and mobile clinic Maintains and facilitates the use of telehealth as needed for clinical and administrative services Completes required records, reporting, forms, and other documentation according to agency and other state and federal guidelines Performs related responsibilities as required or directed Participates in Quality Assurance and Improvement and Utilization Management Guidelines. Completes all reports required by the program Monitors compliance with program guidelines
JOB SPECIFICATIONS:
EDUCATION:
  • Individual must have a bachelor’s degree in Social Work, Psychology or other related field, Masters preferred
LICENSURE, REGISTRATION, CERTIFICATION:
  • Valid driver’s license & ability to obtain any other licensure needed for unit within 30 days
  • Emergency behavior management training within 90 day orientation period and maintained throughout employment.
EXPERIENCE:
  • Previous experience in mental health preferred.
