JOB SUMMARY: The Patient Care Team Representative is an active member of the care team and is primarily responsible for assisting patients with managing phone calls, scheduling appointments, and information management within the electronic health record.

Position is located on-site in Sebastopol and requires some evening (12p-8:30p) and Saturday (8:30a-12p) schedules.

RESPONSABILITIES:

  • Actively participate as an integral and valued member of the office “Care Team.”
  • Answer phone, routes call, and schedule patient appointments.
  • Work toward making patients feel valued and supported within the health center.
  • Must comply with all federal, state and local laws and regulations, and WCHC policies.
  • Help in the management of provider schedules by calling patients the day prior to their scheduled office visits and manage the same day appointments through “Open-Access Scheduling”.
  • Manage information entry into the electronic health record such as faxes, previous medical records, radiology results, and assist with communication with other medical facilities through the electronic health record.
  • Collect and account fees for service.
  • Counsel patients for eligibility programs including Sliding Scale, BCAP, and Family Pact
  • Assist in processing referrals.
  • Assist with clinical data outcome reporting and actively participates in quality improvement projects.
  • Assist is the adoption and implementation of new technologies and activities that improve patient care and clinic efficiencies.
  • Assist other office staff with copying, scanning, faxing, filing, and other general office duties as may be assigned from time to time.
  • Other duties as assigned.

EXPERIENCE & QUALIFICATIONS:

  • Bilingual English/Spanish speaking is required.
  • Experience in an outpatient setting or working with a health clinic desirable.
  • Able to work a flexible schedule including some nights and Saturdays
  • Computer literacy and familiarity with office machines such as fax, copier, calculator.
  • Ability to work as team member.
  • Ability to relate well to varied client base.
  • Ability to maintain good relations with the public and the Health Center staff while working under pressure.
  • Good communications skills
  • Must be able to perform the physical demands of the job.
  • Basic office and equipment safety knowledge and practice.
  • Bilingual Spanish/English is required for this role.
  • Demonstrated commitment and skills to provide age appropriate care to a diverse population

Physical Requirements:

  • Must be able to move freely about the office and carry light loads (up to 25 pounds)
  • Prolonged standing and occasional sitting required.
  • Ability to work with office equipment, including computers
