West Cook YMCA is Hiring a Community Integrated Health Manager Near Oak Park, IL

POSITION SUMMARY

Through our Community Health and Wellness programming and services, the West Cook YMCA delivers evidence-based health interventions designed to reduce health risks, improve health equity, and promote lifestyle change in a supportive environment. The Community Integrated Health Manager (CIH Manager) will provide program support by coordinating participant recruitment, enrollment, member and partner experience, data collection, program fidelity, and Lifestyle Coach support.

ESSENTIAL FUNCTIONS

1. Become certified in all relevant evidence-based health initiative programs.

2. Oversee onboarding process from participant referral to program enrollment, ensuring all referrals are processed correctly.

3. In coordination with the Director, improve communication and ensure reliable and efficient interoperability and enrollment processes.

4. Manage data collection and entry to ensure accuracy while meeting specific billing requirements. Confirm that all new data is entered to be validated by the Director within 48-72 hours of program session and is available to be used for reports. Create a table of participant data for all interventions.

5. Maintain HIPAA compliance and strict patient confidentiality.

6. Complete claims submission and investigate and address any errors in securing reimbursement.

7. Respond to program candidate and referring partner inquiries.

8. With the Director, perform program outreach to facilitate enrollment and program awareness.

9. Execute timely follow-up with referred candidates and waitlisted parties. Plan for, schedule, and conduct enrollment meetings with program candidates to discuss health goals, determine eligibility, perform readiness assessments, ensure paperwork completion, and complete cohort assignment.

10. Manage proper onboarding and communication to program participants, including but not limited to facility tours, wellness consultations, and cohort updates.

11. Realize high-quality program delivery by helping to troubleshoot cohort issues, ensuring that Lifestyle Coaches have necessary program materials and curriculum, and serving as a substitute Lifestyle Coach as needed.

12. Assist in the hiring, onboarding, and training of new Lifestyle Coaches to deliver programs.

13. Maintain extensive knowledge of Community Integrated Health and other YMCA programs to actively refer participants during meetings and social determinants of health screenings.

14. Perform any additional duties assigned by the Director.

REQUIRED SKILLS AND REQUISITES

1. Bachelor’s degree in Public Health, Social Work, or a related field or equivalent preferred. Other applicable education, training, and experience, which provide the knowledge, abilities, and skills necessary to perform effectively in the position will be considered.

2. Minimum 2-3 years of prior supervisory experience.

3. Experience implementing evidence-based program models, including targeting specific or underserved populations, managing referral relationships, and implementing quality assurance and data collection protocols a plus.

4. Understanding of the social determinants of health and ability to relate effectively to diverse groups of people from all social and economic backgrounds.

5. Exceptional communication, organizational, and critical thinking skills.

6. Basic knowledge of health behaviors and challenges of making lifestyle changes.

7. Knowledge of basic computer skills, data entry, and healthcare billing preferred.

8. Certification in CPR/AED and First Aid. Must have or be willing to obtain required training and certification, including evidence-based health initiative instructor trainings.

9. Must have a flexible schedule including ability to work early mornings, in the evenings and/or on weekends.

10. Must be willing to travel locally to community locations to conduct enrollment meetings, participate in recruitment activities, and substitute for Lifestyle Coaches.

11. Must be willing to travel, potentially to other states, to receive program trainings.

12. Bilingual in English and Spanish is preferred but not required.

WORKING CONDITIONS

1. Ability to work at least a 10-15-hour work week that will include weekend/evening hours.

2. Ability to walk, stand, and sit (including on the floor) for long periods of time.

3. Exposure to communicable diseases and bodily fluids.

4. Must be able to lift and carry supplies weighing up to 40 lbs.

5. Ability to stand or sit while maintaining alertness for several hours at a time.

6. Position may require rigorous physical activities, bending, leaning, kneeling, and walking.

7. Visual and auditory ability to respond to critical situations and physical ability to act swiftly in an emergency.

· Total annual compensation $40,000- $45,000 full-time exempt
