  • We offer a combined paid time off (PTO) policy which incorporates state paid sick leave with company paid time off at an accrual rate of 0.0607 per hour worked, equivalent to 120 hours of PTO at 2,080 hours worked in a year. We also offer 6 Paid Holidays (8 hours for full time and 6 hours for part time per event, excludes on-call staff) and 1 Personal Holiday of Choice per year (excludes on-call staff).
  • Retirement planning is encouraged through our 403(b) plan that includes a generous 100% company match on the first 4% of earnings an employee contributes. There is a 5-year vesting schedule on the company match, and minors are ineligible for the company match.
  • A food and beverage discount of 50% is available to all employees at any Wesley bistro. Employees are also eligible for On-Demand Pay with Dayforce Wallet. Minors need parental consent to access this benefit.
  • To assist employees with challenges outside of the workplace, Wesley offers an Employee Assistance Program (EAP), which is 100% company paid. Additionally, Wesley Community Foundation provides grants to qualifying employees as detailed in the plan summary.

Enrich thelives of older adults through community, choice, and continuing care in the HomeHealth License Practical Nurse (LPN) role. Under the direction of aregistered nurse, provide high-quality nursing care to home health and hospice patientswithin the comfort of their homes (on and off retirement community). The LPNwill work closely with patients, families, and other healthcare professionalsto develop and implement individualized care plans. This role requiresexcellent clinical skills, strong communication abilities, and a passion fordelivering personalized care to patients in need. Find your sense of belonging at Wesley!


 You willprovide continuing care through these responsibilities:

  • Conduct follow-up assessments of patients' health status andongoing evaluations to monitor changes in condition.
  • Follow care plans created by theRegistered Nurse or other members of the Interdisciplinary Team (IDT) incollaboration withpatients, families, and healthcare team members.
  • Administer medications, treatments, and procedures as prescribedby physicians.
  • Monitor and record vital signs, symptoms, and other relevantpatient information.
  • Provide education to patients and families on disease management, diseaseprogression, medication administration, and overall wellness.
  • Coordinate with other healthcare professionals, such asphysicians, therapists, and social workers, to ensure continuity of care.
  • Perform wound care, catheter care, and other nursing procedures perthe care plan as established by the Registered Nurse.
  • Assist with activities of daily living, including bathing,dressing, grooming, and mobility.
  • Maintain accurate and up-to-date documentation of patient careactivities and progress notes.
  • Participate in interdisciplinary team meetings to discuss patientprogress, goals, and discharge planning.
  • Adhere to all regulatory standards, policies, and proceduresgoverning home health care services.
  • Provide emotional support and encouragement to patients and theirfamilies throughout the care process.
  • Collaborate with community resources and agencies to facilitateaccess to additional support services for patients as needed.
  • Learn and apply Company, Federal, State and Local rules,regulations, and policies.
  • Other duties and special projects as assigned.

Our requirements and qualifications for success:

  • Completionof a qualified Licensed Practical Nursing (LPN) program and successful passingof the NCLEX.
  • Current Licensed Practical Nurse(LPN) license in Washington State or out-of-state MSL
  • If MSL, follow the Washington Nurse Practice Act.
  • If MSL, complete Suicide Prevention Training in advance of hireper state regulations.
  • If MSL, complete Washington State Board of Nursing (WABON) MSLdemographic survey.
  • Obtain CPR and First Aid Certification within 30 days of hire.
  • Ability to obtain a Washington StateFood Worker Card within 14 days of hire.
  • Effective and comprehendible English verbal and writtencommunication skills to accomplish charting, record keeping, and to comprehendwritten orders and procedures.
  • Displays a personable and comforting personality to care for theresidents.
  • Use logic and reasoning to identify alternative solutions,conclusions, or approaches to problems.

Physical,environmental, and mental requirements:

  • Ability to give direct patient care, including participating inlifting, transferring, positioning, and assistance of residents with walking orin wheelchairs.
  • Ability to lift and move patients as needed.
  • Comfortable working in various home environments, including thosewith potential hazards such as pets or smoking.
  • Capable of standing, walking, bending, and kneeling for extendedperiods.
  • Good manual dexterity and visual acuity for performing nursingprocedures.
  • Ability to listen to and comprehend information and ideas providedthrough conversation, email, and text.
