WES Health System is Hiring a Family Peer Specialist Near Philadelphia, PA

GENERAL OVERVIEW OF KEY ROLES & RESPONSIBILITIES:

The Family Peer Specialist, an adult with lived experience as a family member of a child with behavioral health challenges will provide support to children and their families by engaging with families, incorporating their voice into treatment, and by providing resources as needed.

ESSENTIAL & CORE FUNCTIONS:

1. Provide support to families around IBHS from the perspective of a peer.

2. Empower the family to understand their role as a member of the treatment team

3. Provide support and coaching during meetings

4. Work with families to develop natural supports

5. Provide consumers and their families linkages to new services needed

6. Ensures appropriate referrals to community resources and follow-up on these referrals.

7. Consults with and documents contact with IBHS consumers’ unique caregivers

8. Update contact information in Credible

9. Executes discharge plan by making all referral to community resources.

10. Attend all interagency meetings.

11. Monitors consumers’ behaviors emphasizing prevention rather than intervention.

12. Intervenes as needed when problem behaviors are exhibited, which includes but are not limited to counseling, removing them from the area, verbal prompts, securing the assistance of clinical staff, etc.

13. Prevents crisis situation however when necessary deescalate children in crisis.

14. Inputs outcome data into computer system.

15. Participate in and adhere to Individualize Training Plan

16. Collects outcome data for consumers.

17. Follows up on all consumers needing hospitalization.

18. Completes follow up on discharged IBHS consumers.

19. Identifies and establishes contact with community resources.

20. Completes all paperwork within specified time frames.

21. Completes home and community visits.

22. Represents program/Agency at community activities/fairs.

23. Completes all paperwork within specified time frames.

24. Makes recommendations to HR regarding hiring and termination of staff.

25. Assures continuous quality assurance/program development.

26. Complies with WES standards for service delivery.

27. Meet service productivity expectations.

28. Complete all paperwork within specified time frames.

29. Maintain an understanding of agency policies and procedures.

30. Participate in and adhere to Individualized Training Plan.

31. Attend and participate in supervision in accordance to regulatory standards.

32. Adhere to WES’s Code of Ethics and comply with State Mental Health Code.

33. Attend trainings as required by WES.

34. Participate in continuous quality assurance/program development.

35. Comply with WES standards for service delivery.

36. Maintain consumer confidentiality.

ADDITIONAL RESPONSIBILITIES:

1. Performs other duties and special projects as assigned.

PREREQUISITES & QUALIFICATIONS FOR THE POSITION:

1. A history of involvement with the Behavioral Health System, personally, with a child, or family member.

ORGANIZATIONAL ACCOUNTABILITY & RELATIONSHIPS:

1. The individual is to be supervised by the Clinical or Administrative Director.

COMPETENCIES & PERSONAL CHARACTERISTICS

1. Proficient in computer programs such as Microsoft Office, Excel, Word and PowerPoint.

2. Solid oral and written communication skills including business writing, proper grammar and spelling.

3. Good organization skills including the ability to prioritize work and manage conflicting deadlines.

4. The ability to work well with others.

5. The ability to handle conflicts with diplomacy and tact.

6. The ability to listen and evaluate objectively.

Job Type: Full-time

Pay: $32,000.00 - $38,000.00 per year
