Wellspring Living is Hiring a Human Services Professional (LMSW/LCSW)- Residential Near Union, GA

 

Organization Type: Non-Profit, Faith Based, 50 Employees 

Position Type: Full-Time, Salaried; Exempt 

Reports To: Program Director 

Department: Girl’s Residential Program (Ages 12-17) 

Hours: 40 hours per week; work hours will vary to overlap some early mornings, late evenings and weekends; on call rotation required 

Travel Requirements: Yes 

Education and Experience: 

1. Possess a Master's degree from an accredited college or university in the area of social work or a related field. License- eligible required, associate or fully licensed preferred. 

2. Two years of experience providing services to children and families is preferred. 

3. Experience working in an RWBO facility/program, specifically complying with GA SCORE reporting requirements and documentation strongly preferred. 

4. Experience providing services to children, specifically those who are survivors of commercial sexual exploitation (CSEC) or those exposed to trauma, and training in TF-CBT and Trauma informed care is also preferred. 

Job Summary: 

The Human Services Professional (HSP) is responsible for ensuring that the educational, medical, emotional, and social needs of the child are met and is also responsible for providing and/or coordinating social services for the child. The HSP will oversee the ongoing case management needs of the participants at GRP. The HSP will ensure that Wellspring Living’s Girls Residential Program abides by the RBWO minimum standards. 

Job DutiesThe duties of this position include but are not limited to the following: 

Main Responsibilities: 

● Oversees the ongoing case management needs of the youth in our care 

● Monitors participants’ medical, educational, and social needs; also coordinates appropriate services 

● Ensures RBWO documents are submitted in a timely manner 

● Audits files to ensure RBWO compliance quarterly 

● Monitors participants’ medical and mental health needs and coordinates appropriate services 

● Ensures that all policies and procedures are routinely followed according to the RBWO minimum standards as it relates to case management and files 

● Collaborate with the leadership team. 

Detailed Job Responsibilities: 

● Conduct a minimum of two face-to-face interactions per month with each participant, focusing on assessing and coordinating tailored services to meet identified needs.

● Responsively address case management requests submitted by participants, ensuring timely and effective resolution. 

● Diligently complete and submit essential case management documentation on a monthly basis for all DFCS participants, including General Contacts, Every Child Every Month, and Every Parent Every Month. 

● Provide comprehensive, documented monthly progress updates to legal guardians and crucial external partners on the participant's support team. 

● Skillfully coordinate medical and dental services for participants, including the provision of necessary transportation when needed. 

● Thoroughly document all services in the participant's physical file, Electronic Health Record (EHR), GA SCORE, and GA SHINES. 

● Serve as the primary point of contact for all external partners regarding participant care, fostering effective communication and collaboration. 

● Attend various meetings, including but not limited to Team Review Meetings, weekly clinical meetings, Participant-Focused Meetings, DFCS Pause Calls, and Court Hearings as necessary. 

● Facilitate the discharge process for all participants, ensuring a smooth transition with comprehensive planning. 

● Efficiently coordinate, schedule, and document visits for participants with members of their support team and family members from their safe contacts. 

● Act as a supportive house parent, advocating on behalf of participants and ensuring their well-being. 

● Attend both mandatory and suggested RBWO training to stay informed and enhance professional development. 

● Conduct regular audits of participant general records to ensure compliance with RCCL and OPM regulations, maintaining a high standard of care. 

● Support medication management by coordinating care with providers and pharmacy; inform staff members of medication changes 

Develop and Model Health Living for Participants: 

● Develop intentional connections with the participants 

● Lead by example in modeling a healthy lifestyle 

● Provide direction and encouragement to program participants and staff members 

● Practice and teach healthy boundaries to program participants and staff members

● Assist with providing healthy activities and community for participants 

● Navigate interpersonal conflict by speaking the truth in love 

● Provide support and crisis help as needed 

● Maintain a teachable attitude 

Qualifications: 

● Never have been shown by credible evidence (e.g., a court or jury, a department investigation, or other reliable evidence) to have abused, neglected, sexually exploited, or deprived a child or adult or to have subjected any person to serious injury as a result of intentional or grossly serious injury as a result of intentional or grossly negligent misconduct as evidenced by an oral or written statement to this effect obtained at the time of application and evidence of having made efforts to obtain and evaluate references from previous employers 

● Documentation of at least two professional, educational or personal references that attest to the person’s capability of performing the duties of the position and to the person’s suitability of working with or around children

● Satisfactory preliminary criminal history background check determination and a satisfactory fingerprint records check determination as required by law 

● Clean motor vehicle record is essential to fulfilling the job responsibilities

● Documentation from a licensed physician or other licensed healthcare professional of a health screening examination within thirty (30) days of hiring sufficient in scope to identify conditions that may place residents at risk of infection, injury, or improper care 

● Current evidence of successful completion of a biennial training program in cardiopulmonary resuscitation (CPR) and a triennial training program in first aid which have been offered by a certified or licensed healthcare professional; such programs shall be completed within the first year of employment 

● Willingness to pursue and participate in continuing education to stay up to date on best practices and regulatory compliance requirements 

● If a master’s level candidate, willing to pursue or maintain professional licensure by the state of Georgia 

● Participate in all required trainings and show competency in skill areas of training content 

● Must be 21 years of age or older
