Wellmore, Inc. is Hiring a Clinician - Functional Family Therapy Near Waterbury, CT

 
Background 

The Clinician Case Manager is responsible for direct services with adolescents and their families using a strength-based model; Functional Family Therapy (FFT). FFT is a short-term, high-quality intervention program with an average of 12 to 14 sessions over approximately four months. FFT works primarily with 11- to 18-year-old youth who have been referred for behavioral or emotional problems by the juvenile justice, mental health, school, or child welfare systems. Services at Wellmore are conducted in the home with all family members present. 

At its core, there is a focus on assessment and intervention to address risk and protective factors within and outside of the family that impacts the adolescent and their adaptive development.

Responsibilities 

1. To provide comprehensive evaluations, treatment plans and treatment of adolescents and their families utilizing Functional Family Therapy. This includes evaluation/assessment; home and community-based services; outreach; case and care management and family therapies.
2. To implement treatment plans that may include psychotherapy and clinically required support services including but not limited to translation, transportation (only as per agency policy) advocacy, and linkage to other needed services.
3. To provide community-based outreach and treatment services as defined by the treatment plan and/or client need, in a variety of settings including but not limited to: agency offices sites, client homes, schools, shelters, police stations, etc.
4. Maintain contact and provide follow-up with families requiring this service. This will include daily home visits and home-based therapies.
5. Maintain confidential and/or any other required program documents.
6. Maintain a level of productivity as assigned by the program supervisor.
7. To work collaboratively as a member of a team with other professionals and concerned parties involved in cases.
8. To assist in the organization, marketing, planning and implementation of the program.
9. Actively participate in FFT and internal training and staff meetings.

 Requirements 

Master’s Degree in marriage and family therapy, social work or related human services field with at least a one-year clinical internship working with children and/or adolescents or families is required. Must hold one of the following credentials to start employment: LMSW, LCSW, LMFT, LPCA, LPC, or licensed or license-eligible psychologist in Connecticut. 

Two years of post-graduate clinical experience working with children, adolescents or families is preferred. 

Experience in family therapies as well as family systems theory and practice is preferred.

Extensive training is provided.
