POSITION TITLE: Case Manager

PROGRAM: DCF Supportive Housing Program


GENERAL STATEMENT OF RESPONSIBILITIES:

This position involves the provision of case management services to clients and families referred from the Department of Children and Families through linkage to Wellmore Behavioral Health’s collaborator in this program, The Connection, Inc. The Case Manager does intake assessments and develops an individualized care plan for each member of a family. This plan addresses housing needs particularly but also provides for referrals to address social service and financial needs and the medical, psychological and counseling needs of family members. The Case Manager reviews the efficacy of and monitors client compliance with the care plan via home visits and collateral contacts. The Case Manager works collaboratively with the assigned DCF social worker to ensure the quality of life and safety of children. The position also requires producing client reports for team review and the maintenance of up-to-date client records per policies of The Connection, Inc. Monthly statistical reports are produced for tracking of activity and supplied to Wellmore Behavioral Health.

SUPERVISION: Supervision is provided in staff meetings, case review, and through direct observation.


EXAMPLES OF RESPONSIBILITIES:

1. Abides by the Code of Ethics of Wellmore Behavioral Health.
2. Completes intake and client assessments to determine eligibility for inclusion in program.
3. Develops an individualized plan of services for client, incorporating client input. Reviews and revises this plan at intervals and as needed. Monitors client compliance with care plan
4. Coordinates team review meetings with DCF social worker, substance abuse and/or other treatment providers and other team members/providers as identified in service plan.
5. Coordinates client referrals, provides advocacy, crisis intervention and counseling services to clients and families as needed.
6. Maintains up to date records as mandated by Wellmore Behavioral Health and The Connection, Inc. Documents client progress and submits reports to the case review team maintaining confidentiality of all records.
7. Works directly with the DCF social worker to ensure the safety and stability of children, including unannounced home visits and on-call coverage.
8. Provides a full range of case management services to program participants. This may include coordination of the following services: substance abuse recovery, psychological/psychiatric treatment, relapse prevention, children's services, educational support for parent and/or children, parenting training, child care, child development training, life skills training, health education, educational and vocational training, financial management, job development and placement training, home management training and transportation.
9. Establishes and maintains a network of landlords throughout the region and makes individualized housing placements for clients, maintaining high standards for both landlord security and client comfort.
10. Participates in ongoing agency trainings and staff meetings at The Connection, Inc. and completes required training requirements and attends relevant staff/supervisory meetings at Wellmore Behavioral Health.

QUALIFICATIONS:

EXPERIENCE:
Bachelor's Degree in a human services-related field or relevant experience in human services, preferably encompassing experience in child welfare, substance abuse, welfare or the supportive housing system.

LICENSURE/CERTIFICATION/REGISTRATION:
CPR and First Aid Certification. Must have own transportation, valid driver's license, proof of active auto insurance, and a driving record that allows for inclusion on Wellmore Behavioral Health policy.
