POSITION TITLE: Team Leader (Program Supervisor)

PROGRAM: Community Support for Families

GENERAL STATEMENT OF RESPONSIBILITIES:

The Team Leader for Community Support for Families is responsible for the day to day management of all Wellmore, Inc. Community Support for Families services. She/he ensures quality, culturally responsive services are provided to children and their families which is consistent with the core values and guiding principles of the Strengthening Families and WRAP CT principles. He/she trains and supervises agency Community Support Workers. The Team Leader establishes and implements referral, screening and case assignment processes that facilitate rapid access to services for families with children who meet program eligibility requirements. She/he represents Wellmore, Inc. at the local DCF Area offices throughout the region and collaborates with the related Investigations Units to improve access and outcomes for families eligible for a Differential Response track. He/she carries a caseload as necessary and works a flexible schedule based on family needs. This may include evenings, weekends and/or holidays.

Supervised by: CSF Project Coordinator, the Director of Home Based Services or designee

Supervises: Community Support for Families staff


EXAMPLES OF RESPONSIBILITIES:

  • To oversee the day to day operations of Wellmore Community Support for Families (CSF) program.
  • To train and supervise agency CSF staff.
  • To ensure quality, culturally responsive services are provided to children and their families which is consistent with the core values and guiding principles of the Strengthening Families and WRAP CT principles.
  • To establish and implement referral, screening and case assignment processes that facilitate rapid access to CSF for families who meet program eligibility requirements
  • To serve as a bridge to services for eligible families that enter DCF Investigations Unit Differential Response (DRI) track and are appropriate for CSF services.
  • To assist the CSF Project Coordinator in representing Wellmore, Inc. at the Differential Response Implementation meetings throughout the region and collaborates with applicable systems to continually improve outcomes for families eligible for DRI.
  • To carry a caseload as necessary, providing service brokering, linkages, coordination, follow-up and advocacy.
  • To perform community outreach in order to educate the community about Community Support for Families and to help promote DRI as assigned by the CSF Project Coordinator or designee.
  • To perform utilization review (chart reviews, monitoring caseloads, length of stay, etc.) and other quality management functions.
  • To insure all program records and reporting are timely, accurate and consistent with agency standards and the program contract.
  • To solicit feedback from the community, providers and consumers of CSF services as a means of continually improving the service.
  • To insure the program meets agency and contractual targets and requirements and to routinely update the Supervisor regarding program performance.
  • To represent the agency and program at assigned local and statewide forums as needed.
  • Other such duties as assigned by the Supervisor.


QUALIFICATIONS:

Education and Experience:

Successful record in nonprofit staff supervision (including the ability to meet program and fiscal targets). The ability to work independently, see projects through from start to completion, motivate/mentor and encourage/challenge staff while focusing on quality outcomes, as well as the ability to function effectively as a member of a management team, are also necessary. Must understand and be able to display the knowledge and expertise necessary to perform supervision of mental health professionals. Must be culturally competent to serve agency clientele. Must be able to effectively communicate with staff, client families, Board members, other professionals and the community at large. An ability and comfort with public speaking on topics relevant to the Wellmore, Inc. mission. Bi-lingual (Spanish-English) may be required.

The ideal candidate is: Capable of creative problem solving skills; a self initiator, with high energy and good listening skills; has excellent interpersonal skills and is knowledgeable of the area we serve.

A Master’s degree and an active CT license with the Dept. of Public Health as a Marriage and Family Therapist, Professional Counselor or Social Worker is required (LMFT, LPC, or LCSW). A master’s degree in a human service field (LMSW, LPCA, or LMFTA ) with at least one (1) year clinical internship working with children and/or adolescents will be consider; Must be eligible for a CT License as a LPC, LCSW, or LMFT.

Must have at least 3 years of experience in human services with progressive responsibilities and supervision of staff.

Must have knowledge of cultural diversity and be capable of displaying cultural competence to the populations served by the program.

Must be capable of working as a part of a team and be able to effectively communicate with both families and professionals (in writing and verbally). Bi-lingual (Spanish-English) may be required.

The successful candidate must have the ability to maintain the utmost confidentiality. Must be capable of both working independently and as part of a team.


LICENSURE/CERTIFICATION/REGISTRATION:

Valid driver’s license with good driving record. Must have reliable, personal transportation and proof of auto insurance is required.
