Job Description

The housing coach is part of a team that develops and executes housing navigation and placement services including follow-up post placement for referred clients. Together with the transitional coordinator, the housing coach will support the client in understanding the process, navigating eligibility, preparing, and completing the move and being a consistent supportive resource.

Essential Accountabilities:

  1. Two days a week in the office. 3 days per week of fieldwork or remote work or as otherwise specified by the Director of Intake Services.
  2. Provide information and resources to clients during all phases of the housing search.
  3. Provide guidance with the application process and assist with completing paperwork and housing applications as needed.
  4. Coaches clients for housing interviews
  5. Accompany client on site visits
  6. Act as client’s support and advocate throughout the entire process.
  7. Provide escort to site visits and other appointments as required.
  8. Provide accurate, complete, and timely data entry and other documentation of all interactions and assessments with clients.
  9. Assist the client with activities of daily living if needed (e.g., setting-up household, public transportation, budgeting, shopping, socializing, parenting and other skills relevant to an individual’s rehabilitation goals). May make home visits.
  10. Attend staff meetings, case conferences, group supervision, and professional development activities such as in-service trainings, seminars, webinars, and conferences as needed. Work with other consortium partners as necessary.
  11. Support an environment that embeds diversity, equity, inclusion, and accessibility (DEI)
  12. Participate in workplace culture that inspires high-level performance, collaboration, accountability, and professional and respectful interaction.
  13. Employ best practices for effective feedback and communication at all levels of staff engagement.
  14. Perform all job-related tasks as required.

Other Responsibilities:

  1. Make community contacts with possible referral sources, clinics, psychiatrists, day programs, etc.
  2. Make presentations at various outside agencies on an as needed basis.
  3. Maintains confidentiality at all times.
  4. Work locations and schedules are subject to change.
  5. Any additional duties as assigned by supervisor.
