As our Outreach Associate (hybrid remote/in-person), you will focus on proactive community outreach efforts and collaborate with our referral partners to help uncover and deliver resources to underserved populations. Your ability to work quickly, build rapport, and exceptional communication skills will be crucial.

Minimum Qualifications

• Consider yourself a "people person" and enjoy connecting with others
• Proficiency in computer technology such as screensharing, creating and editing PDFs and Word documents, navigating insurance platforms and using multiple software systems simultaneously
• Maintain regular and consistent attendance, punctuality and communication
• Completion of both Indeed skills tests

What you'll likely work on

• Regularly participate in virtual and in-person community resource and tabling events
• Foster relationships with social, medical and financial programs that benefit underserved populations
• Connect patients with community resources not only relating to mental health but also homelessness, financial assistance and hunger
• Maintain solid relationships with our referral partners online and offline, ensuring proper sharing of patient data and continuity of care
• Contribute to a tight-knit team dynamic--we mostly work remotely but collaborate closely by working together online and talking to each other every day to tackle tasks efficiently and effectively
