We are seeking an Outpatient Clinical Program Therapist to join the team at WellBridge Healthcare Fort Worth!

Job Summary:

 The Behavioral Health Outpatient Clinical Program Therapist is responsible for the overall management of the program milieu. She or he will participate in Quality and Risk Management functions such as reporting data as required to state agencies and hospital committees, conducting audits, conducting compliance rounds, and ensuring the safe and effective delivery of care.

The Outpatient Clinical Program Therapist ensures that office operations and programing in the outpatient department operate efficiently and professionally while providing a high standard of customer satisfaction. Ensures schedule adherence and outpatient programs are compliant with all laws, rules and regulations of federal and state licensing agencies, as well as TJC standards for quality care.

Assists with monitoring patient volumes and quality of care to identify areas for improvement.

This role assists in the implementation of outpatient care programs, policies and procedures that dictate how patients’ care needs are assessed and met in the specified programs and services. This role includes responsibility for: ensuring standards for patient care are maintained and provide a consistent standard of care to all participants; Implementing and monitoring an effective, ongoing program to measure, assess and improve the quality of care delivered to participants; assisting with and promoting marketing, community relations related to the specified units and services; and assisting in expanding the continuum of services by exploring market needs and opportunities and responding with new program design, implementation and evaluation. May provide services for various patient populations including children, adolescent, adult and geriatric.

  • Ability to work independently in a fast-paced outpatient environment
  • Ability to have flexible hours in order to meet business needs.
  • Ability to cultivate and maintain relationships with internal and external customers as well as maintains patient/family confidentiality and provides sensitive, respectful and compassionate customer service at all times.
  • Ability to work with children, adolescents or adults (18 and over) experiencing mental illness and acute psychiatric crisis.
  • Must demonstrate good verbal and written communication skills.
  • Ability to coordinate the transfer of patients to other programs.
  • Assists in maintains monthly reports on statistical information on program compliance.
  • Manages and responds to incoming phone and fax inquires by maintaining data regarding demographic information on potential referrals, and information within establishes turnaround timeframes.
  • Develops an understanding of responsibilities for participation in Quality Improvement activities.
  • Assumes personal responsibility for obtaining and maintaining professional development and education. Maintains or obtains relevant licensure as appropriate
  • Demonstrates leadership in planning programs/services, interventions, policies, plans of correction, and management of staff. Initiates and pursues program ideas, responding to community/market need, regulatory changes, patient service needs and quality service demands.
  • Demonstrates proficiency and skill in program direction as exhibited by:
    • Coordinates operations of services to assure the program philosophy is implemented; including recruitment of key positions to assure adequate professionals are available to achieve this objective.
    • Ensures program compliance with all federal and state regulations. Stays abreast of regulatory changes impacting the provision of care and continually monitors CMS, TJC, and other prevailing governing bodies for most current guidance and trends. Modifies the program on an ongoing basis to ensure full compliance with statutes as well as optimal patient outcomes.
    • Attends treatment team meetings on a regular basis.
    • Ensures that clarification of the therapeutic role and treatment plan with the patient and family is completed and reviewed by each therapist.
    • Actively participates in the complaint resolution process as needed.
    • Actively seeks to identify new potential referral sources and maintains good working relationships with all referral sources.

 Knowledge/Skills/Abilities/Expectations:

  • Excellent oral and written communication and interpersonal skills
  • Must have good and regular attendance.

  • Performs other related duties as assigned.
  • Knowledge of basic word-processing and spreadsheet software required.
  • Good typing skills and familiarity with medical terminology.
  • Ability to work independently.
  • Prompt, efficient, accurate handling of referrals and admissions.
Qualifications
  • CPR Certified and able to perform CPR techniques
  • CPI certified and able to complete physical management training and perform physical management techniques.
  • Demonstrates strong critical thinking skills, diplomacy, and relationship building.
  • Ability to interact with a wide variety of individuals and handle complex situations.
  • Excellent written and verbal communication skills are required.
  • Attention to detail and timely follow up.
  • Knowledge of and commitment to the mission, philosophy, objectives and Core Six Values of Kindred Healthcare.
  • Must read, write and speak fluent English.

 Education:

  • Minimum Master’s Degree in Social Work, Marriage and Family Therapy, or Professional Counseling

Licenses/Certification:

  • LMSW, LCSW, LMFT, LPC, LPC-I

 Experience:

  • Minimum of 3 years’ experience in a psychiatric setting. Experience in Therapeutic verbal de-escalation techniques.
