Job Summary

WELLBE INTRODUCTION

The WellBe care model is a Physician Led Advanced Practice geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients. This population is typically underserved and very challenged with access to care. To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. Care is provided throughout the entire continuum of care – from chronic care and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative care, to end of life care. WellBe's physician/advanced practicing clinician led geriatric care teams’ partner with the patient’s primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum.

GENERAL SUMMARY

This role will advance WellBe’s understanding of our patient's healthcare utilization and how we can reduce unnecessary healthcare costs while achieving better patient outcomes, and patient experience. The ideal candidate will have a solid knowledge/understanding of healthcare data sets, including medical claims, HCC, HEDIS, and be detail oriented. This role reports to the Chief Executive Office and works closely with operational, IT and clinical leaders. Responsibilities include collaborating with finance, operational, IT and clinical leaders to design analyses to answer key questions about our care model, complete in-depth analyses leveraging medical claims, EMR inputs, patient attributions, and healthcare operations data sets. Identify key insights and communicate to various stakeholders through the WellBe organization.

Job Description

SKILLS & COMPETENCIES

  • Responsible for analytic data needs of various business unit.
  • Handle complex data projects and acts as a lead for other Data Analysts.
  • Provide advanced analytical support for business operations in all or some of the following areas: claims, provider data, member data, clinical data, HEDIS, pharmacy, external reporting
  • Extract, load, model, and reconcile large amounts of data across multiple system platforms and sources
  • Review data to determine operational impacts and needed actions; elevate issues, trends, areas for improvement and opportunities to management.
  • Develop reports and deliverables for management
  • Ensure compliance with federal and state deliverable reporting requirements by performing data quality audits and analysis.
  • Assist with training and mentoring other Data Analysts.
  • Analyze medical cost and utilization trends is support of medical economics and population health initiatives supporting our ACO
  • Work with key business owners to establish and meet their analytic needs and the analytic needs of the organization
  • Execute the various outcome and predictive analytic studies that support MHN’s model of care
  • Complete a variety of clinical and financial client ad hoc analyses in an efficient and timely manner.
  • Developing and using risk models for comparative analysis and patient stratification
  • Work with IT teams to operationalize analytic and reporting innovations
  • Conduct analysis to support Shared Savings and other key improvement programs
  • Develop actionable dashboards and reports to support care management and ACO operations
  • Management, maintenance, and design of integration strategies regarding eligibility and empanelment including data quality, reporting and retention initiatives.
  • Creates, develops and maintains reports, datasets, ad hoc requests, dashboards, metrics, data generators, etc. for end-users using system tools and databases.
  • Produce actionable reports that show key performance indicators, identify areas of improvement into current operations, and display root cause analysis of problems.
  • Analyzes and determines data needs for end-users.
  • Accesses, extracts and integrates data from diverse sources residing on multiple platforms and implement data models by combining, synthesizing and structuring data.
  • Other tasks needed to accomplish team’s objectives/goals

Job Requirements

Educational/Experience Requirements:

  • Bachelor’s degree in related field or equivalent experience.
  • Healthcare experience required.
  • Minimum 4 years of professional experience in a data analytics role within healthcare (payer, provider, consulting)

Required Skills and Abilities:

  • Advanced knowledge of Enterprise Reporting and Analysis tools, SQL, and visual analytic applications.
  • Proven experience, ideally in healthcare, with granular data analysis, predictive analytics or analytics technology products. Hands-on experience reading/writing code in a data analyst role.
  • Fluency in SQL or other similar programming languages. Some development experience in at least one scripting language (Python, R, etc.) is a plus.
  • Experience launching internal metric dashboards and scorecards.

Work Conditions: Ability to lift up to 20lbs. Moving lifting or transferring of patients may involve lifting of up to 50lbs as well as assist with weights of more than 100lbs.
