JOB PURPOSE:

The Home Study and Post-Release Care Manager L3 is responsible for delivering intensive comprehensive case management and supportive services for the Home Study & Post-Release Services program for unaccompanied children who have been released from federal custody. This role involves completing frequent in-person home visits, and documenting findings. This role involves close collaboration with program staff and supervisors to establish and maintain a client-centered, trauma-informed approach in service provisions.

Additionally, the Care Manager L3 continually assesses changes in behavior and conditions impacting the safety and well-being of children, providing crisis intervention and support as needed.

The Care Manager L3 conducts Home Studies to determine a potential sponsor’s ability to meet an unaccompanied child’s needs upon release from Office of Refugee Resettlement (ORR) custody, and Post Release Services which help unaccompanied children and sponsors meet their needs.

The Care Manager L3 provides continuous intensive case management for unaccompanied children, focusing on screening and making appropriate referrals to address various needs, including placement issues, safety concerns, education, health, mental health, legal/immigration needs, substance abuse, independent living, and child enrichment activities.

ESSENTIAL JOB RESPONSIBILTIES:

· Conduct frequent in-home assessments (home-studies) through home visits to identify the individual needs and challenges of unaccompanied children

· Develop and implement comprehensive, individualized case plans that address both immediate and long-term goals

· Provide specific support to complex and vulnerable cases

· Provides crisis intervention services as needed by connecting children and their families with a range of supportive services such as education, healthcare, legal assistance, mental healthcare, etc.

· Provide frequent and comprehensive reports on individual cases, highlighting achievements, challenges, and areas for improvement reporting any significant developments or concerns that may impact the child's well-being

· Design and implement strategies to help children and families adapt to the local culture

· Work collaboratively with program staff, supervisors, and external agencies to ensure a coordinated and holistic approach to service delivery

· Advocate for access to resources that enhance stability and self-sufficiency

· Foster a client-centered approach in all interactions, prioritizing the dignity of each child and family

· Provide emotional support, recognizing and addressing the potential impact of trauma on the child's well-being

· Comply with the policies and procedures of We Speak Your Name of Central Florida (WSYN) and adhere to policies and procedures as set forth by the U.S. Office of Refugee Resettlement (ORR) and the U.S. Conference of Catholic Bishops (USCCB)

· Meet or exceed program productivity expectations on a regular and sustained basis

· Other duties as assigned

QUALIFICATIONS & REQUIREMENTS:

· Master’s degree in human services, social work or a related field

· Must be bilingual in Spanish/English, multilingual in Creole is a plus

· Previous experience in reunification and case management, preferably with immigrant or refugee populations

· Strong assessment and crisis intervention skills

· Knowledge of community resources and ability to connect families to community services

· Excellent verbal and written communication skills

· Maintain a professional demeanor and conduct in all interactions and engagements

· Minimum of 5 years of relevant progressive experience in intensive case management with a focus on working with vulnerable populations

· Demonstrate proficiency in using case management software and relevant applications to maintain accurate and up-to-date case records

· Ability to work with diverse populations and maintain cultural sensitivity

· Must possess a valid driver's license, access to an insured reliable vehicle. Able and willing to travel up to 75% of the time to home visits

· Must be able to complete/pass FBI background check and Child Welfare background check for the last 5 years

Job Type: Full-time

Pay: $60,213.00 - $70,000.00 per year
