Job Summary

Foltsbrook Outpatient Therapy Department has immediate positions available at Foltsbrook Center Nursing & Rehabilitation on 104 N. Washington St, Herkimer, NY 13350 for Outpatient Certified Occupational Therapist Assistant, Per Diem.

A qualified Certified Occupational Therapist Assistant would be responsible for implementing direct occupational therapy services to patients, including treatment, program planning and implementation, related documentation, and communication under physician's orders.

Responsibilities

• Perform appropriate treatment relative to established functional goals.
• Complete daily electronic SOAP note.
• Maintain accurate records of patient treatments.
• Assist in educating patients with home exercise programs and use of equipment.

Requirements

• Graduate of an accredited Occupational Therapy Assistant program.
• Certification as Occupational Therapy Assistant in the state of NY.
• Knowledge of principles, methods, and equipment used in occupational therapy.
