Description

The Case Manager is responsible for providing case management (service coordination; record maintenance; ISP/DHSP paperwork, implementation of ISP/DHSP goals/support plan goals/career plan goals and objectives, maintenance of progress reports/summaries; development and maintenance of regular assessments and career plans where applicable; maintenance of HCSIS related documentation and submissions; maintenance of documentation related to behavior support/treatment plans and guidelines; timecard and attendance) to people receiving services at Waystone Supports. Services will be provided at Waystone Health & Human Services and in the community including: job development, coaching on community activities including jobs/volunteer activities, transportation, travel training. Training regarding proper support of persons served will also be provided by the Case Manager to other staff members as needed/requested by the Director of Community Learning. A commitment to the inclusion of people with developmental disabilities in their communities as valued employees and citizens is required.

LOCATION: Parkridge, Haverhill, MA

SCHEDULE: 40 Hours, Monday - Friday, 8:00 am - 4:00 pm

RESPONSIBLITITES

  • Coordinate service delivery for persons served, in conjunction with Director of Community Learning. Coordinate/attend/maintain notes for all case conferences, support plan meetings, quarterly progress meetings, trainings, etc. related to persons served on caseload. Ensure services provided (paid work, volunteer, community activities, skill-based activities, behavior strategies) meet the goals, objectives, strategies outlined in each person’s support plans (ISP, DHSP, Career Plan, Behavior Support/Treatment plan/guidelines) and promote choice, inclusion, dignity, independence and skill development. Proactively identify concerns and solutions regarding service delivery and work in conjunction with Director and Manager to ensure consistent, effective, and person-driven supports for persons served.
  • Provide personal care, assistance, and support to individuals consistent with their medical, physical, and psychological needs in a respectful manner. Understand/implement crisis plans, behavior treatment plans, and medical protocols correctly and consistently.
  • Complete all documentation including attendance records required by Waystone HHS, funding sources, and regulatory agencies, focusing on providing complete, relevant, accurate, and timely documentation. Participate in the development and implementation of ISPs/DHPS, ensuring that documentation is submitting in a timely manner through HCSIS or other mechanisms (outlined by funding source), in accordance with regulations and Waystone policies. Ensure support plan goals, objectives, strategies are developed which enable individuals to increase their self-sufficiency and ability to fully participate in activities of their choosing.
  • Maintain customer focus at all times in the delivery of services. Foster an environment which is committed to quality, responsiveness to customer needs, and professionalism in customer relationships. Work with employers, funding sources, families, and regulatory agencies in a positive and professional manner to establish support and build strong relationships. Work to increase natural supports and decrease level and intensity of supports offered by Waystone HHS.
  • Assist in the creation, completion and implementation of relevant assessments, career plans, assistive technology plans, and other required documentation as determined by the Director of Community Learning to meet DDS and OQE standards as well as to meet team goals and department initiatives. Complete job site check in’s, job coaching, employer communication, 1:1 meetings with clients, community outings, and facilitate small group sessions as determined by the Director of Vocational Services and Department Team.
  • Perform all duties in a manner which demonstrates an awareness and respect of each individual’s legal, civic and human rights. Maintain and enforce the confidentiality of any information relating to individuals served.
  • Complete all mandatory trainings to ensure skill maintenance and development. This includes CPR, First Aid, and other trainings mandated by Waystone HHS, as well as other department and state agency required trainings and certifications. Complete supplementary trainings related to functions within the Case Manager role, as well as new initiatives identified by Waystone HHS or state agencies.
  • Provide transportation as requested.

Qualifications


QUALIFICATIONS

  • Bachelor’s degree with 30 semester hours or equivalent in a human services field and at least one year of experience in the delivery of services to individuals with developmental disabilities. Commensurate experience may be substituted for educational requirements.
  • Computer literacy and exceptional organizational, writing, time management, presentation and people skills are necessary. Proficiency in Microsoft Word and Excel applications.
  • Minimum of 1 year of driving experience and a valid driver’s license.
  • Regular and predictable attendance
