Compensation:

In accordance with California law, the expected salary range for this position is posted with this position. The actual compensation will be determined based on experience and other factors permitted by law.

Role Summary:

The primary purpose of this position is to act as an advocate for Wayfinder’s group home residents and their families, and to ensure that the medically related, emotional and social service needs of the clients are maintained on an individual basis.

Major Duties and Responsibilities:

  • Act as liaison between residents, families, service providers and the community including medical, psychological, educational and other service resources.
  • Perform social case history on each individual entering the Group Home and write reports as required.
  • Assist and review group home placement referrals.
  • Assist with Group Home intakes and discharges and complete 30-day Individual Program Plan (IPP), Needs and Service Plans, and Discharge Summary reports in a timely manner.
  • Provide consultation to Group Home staff in order to aide in preventing, identifying and treating personal and social problems. Attend daily Group Home staff shift change meetings to facilitate communication of the residents’ needs.
  • Provide direct intervention case and group work with residents and families in order to assist the individual to live as independently as possible.
  • Actively involved in enhancing the coping capacity of the resident’s family. Safeguards the human and civil rights of residents and fosters the human dignity of each individual.
  • Monitor and ensure IPP plans and treatment plans for all residents are being met. Maintain the IPP database and provide outcome reports as necessary to meet Quarterly Report etc. deadlines. Report resident progress during the weekly Group Home meetings and as requested.
  • Responsible for overseeing the Group Home clinical files. Ensures all necessary documentation is in place.
  • Ensure that all required and necessary documentation is completed, including but not limited to, daily communication logs to the schools, weekly team meeting reports, daily meal counts, incident reports, seizure reports, nursing referrals, room check lists, home base check lists, bed check reports etc.

Qualifications


Education/Certifications:

  • Bachelor’s degree in a related field from an accredited college or university is required.

Experience:

  • Two (2) years’ experience working as a case manager, preferred.
  • Knowledge of location regulations, state laws, and procedures.
  • Knowledge of social casework principles and methods.
  • Considerable knowledge of the Regional Center systems, and SSI and Medi-Cal resources.
  • Minimum one (1) year of experience making effective use of community resources, establishing and maintain an effective working relationship with clients, families, staff and outside agencies.
  • Previous experience working in a residential setting, preferred.
  • Valid CA Driver License with a minimum of two (2) years good driving history and must be approved by the agency’s insurance carrier to drive agency vehicles.
