Waupaca County (Wisconsin) is Hiring a Crisis Case Manager - Behavioral Health Near Waupaca, WI

As a function of the Waupaca County Department of Human Services Department, the Crisis Case Manager performs functions that provide case management services to those consumers experiencing mental health crises. This position will provide crisis intervention, de-escalation and case management services to individuals who are currently, or recently have, experienced a psychiatric crisis.

This position works under the direct supervision of Behavioral Health Manager of Wraparound Treatment Services CSP and CCS Programs and Behavioral Health Manager of Outpatient Treatment Services. 

This position will:

Provide mobile emergency mental health crisis coverage which includes assessment, intervention, de-escalation, coordination and referral on an on-call rotation including nights and weekends.

Coordinate court procedures with the Corporation Counsel on case of Chapter 51 detentions. Monitor commitment orders and makes recommendations to the court about the consumer’s compliance and need for further court involvement. 

Conduct intake activities and screens as assigned to provide a crisis plan that outlines current personal issues and what services are needed to provide support and safety in a community setting. Recommends placement options for the consumer as needed. 

Provide case management services to help eliminate current gaps in treatment that will assist the consumer in being more successful in treatment compliance and avoiding the need for re-hospitalization. 

Transport consumers to appointments, interviews, tours of service locations, etc. 

Meet regularly with community services providers to coordinate and monitor client progress; this could include psychiatrist, psychotherapist, employers, crisis facilities, legal system, AODA counselors, day service providers and housing authorities.

Emergency/mental health crisis coverage. 

Attends staff meetings and trainings as directed. 

PERIPHERAL RESPONSIBILITIES:

Performs other duties as assigned by the Behavioral Health Unit Manager.

EDUCATION: Graduation from an accredited college with a degree in social work or human service related field.

EXPERIENCE:  Experience working with the assigned population in need of available service.

Basic knowledge of mental health laws, system and treatment including Chapters DHS 34 and 51

Basic understanding of community social work that includes a person centered and strength based approach. Knowledge of trauma informed care and the impacts of adversities. 

Ability to build alliances among people to provide services outside of institutions.

Ability to maintain accurate and detailed records. 

Ability to follow written and oral instructions. 

Ability to communicate effectively orally and in writing. 

Ability to establish and maintain effective working relationships. 

Ability to maintain agency confidentiality. 

Must have available transportation to perform on-site responsibilities.
