Job Description

Job Title: Clinic Nurse – LPN or RN

Reports to: Clinic Manager

Status: Non-Exempt Hourly

Hours: Full Time & Part Time Positions Available

Summary

The Clinic Nurse supports the administration of general primary care and specialty clinics at the Waukesha Free Clinic, serving the uninsured, underinsured, and low-income folks who live, work, or attend school in the Waukesha County community. We provide healthcare to those who otherwise would not be able to afford or access it, which means we also spend a significant amount of time supporting our patients with assistance related to their social determinants of health. Familiarity with the Waukesha County community and its resources is a plus in this role, but what is most important is a passion for our mission of helping people who cannot get help elsewhere. All duties are done under the supervision of the Clinic Manager (RN).

Major Responsibilities

Clinic Support

  • Plan and manage patient care according to individual patient’s needs
  • Support primary and specialty care clinics and providers from set-up to clean-up
  • Escort patients to their rooms and obtain vital signs
  • Interview patients and record medical history
  • Administer medications as directed
  • Promptly and accurately update patient charts
  • Follow-up on patient lab/diagnostic testing and referrals

Case Management

  • In collaboration with the Clinic Operations Coordinator or other RN staff:
  • Provide comprehensive and intense support services to homeless individuals with chronic and/or multiple health conditions, especially those with physical, psycho-social, and/or environmental barriers to managing their medical conditions
  • Link clients with health care and other services to ensure timely, coordinated access to medically appropriate levels of health services
  • Address continuity of care through ongoing assessment of client
  • With oversight from the Clinic Operations Coordinator or other RN staff, the Clinic and Community Outreach Nurse will:
  • Complete initial needs assessments
  • Develop a comprehensive individualized service plan
  • Coordinate services required to address the service plan
  • Educate clients on treatment adherence and monitoring to assess the efficacy of the plan
  • Collaborate with other providers, for example contacting primary care physicians to notify them of changes in condition or making cross-referrals to community service providers
  • Maintain complete and accurate client records.
  • Provide other records or data as directed by databases
  • Other duties as assigned

Specialized Knowledge and Experience

  • Licensed Practical Nurse with experience in a community clinic or other community health setting. Must hold a current LPN issued by the state of Wisconsin. Candidates with a valid RN license issued by the state of Wisconsin will also be considered for this position.
  • Perform duties in accordance with the Nursing Code of Ethics and the Nurse Practice Act of the State of Wisconsin. Refers all conditions not licensed or skilled to treat to appropriate persons or agencies.
  • Experience and understanding of community-based services – awareness of community resources and benefit programs.
  • Experience working with minority populations – ability to speak Spanish is a valuable asset
  • Familiarity with basic computer functions (e.g. excel spreadsheets, databases, etc.)
  • Meets core competencies within three months of hire

Job Types: Full-time, Part-time

Pay: $25.00 - $33.00 per hour

Expected hours: 8 – 40 per week
