Washington Regional Medical System is Hiring a Registered Nurse (RN) Clinical Liaison - Hospice Near Fayetteville, AR

Position Summary

The role of the RN Clinical Liaison – Hospice reports to the Hospice Clinical Manager in partnership with the Administrative Manager and Vice President. This position is responsible for the intake of referrals and admissions effectively and timely to meet the needs of patients and families. This position effectively collaborates with appropriate interdisciplinary team members to establish a plan of care and meet patient needs.


Essential Position Responsibilities

  • Perform intakes of new referrals and coordinate admissions
  • Coordinate hospice informational visits and evaluation for hospice eligibility
  • Participate in administrative on-call rotation
  • Complete regular follow-ups via phone call or in person visits for pending referrals
  • Maintain comprehensive working knowledge in the field to serve as a liaison for marketing and share information with appropriate community referral resources
  • Maintain partial out-patient hospice case load
  • Perform a comprehensive and accurate patient assessment using evidence-based assessment tools
  • Identify changes in patient conditions and report to appropriate providers and members of the care team
  • Manage and maintain patient medical records through complete and accurate documentation
  • Evaluate patient progress toward expected clinical outcomes, including treatments and health education, in collaboration with the patient and the health care team.
  • Assist with administrative tasks


Qualifications

  • Education: Must be a graduate of an accredited nursing program
  • Licensure and Certifications: Registered Nurse License and BLS, required. Valid driver’s license and auto insurance, required.
  • Experience: 2 years of nursing experience, required.


Work Environment
: This position will spend 60% of time sitting while performing work on a computer or at a desk and 40% of time standing and/or walking while pushing, pulling, lifting, and/or carrying up to 50 lbs.
