WaltonWood is Hiring a Wellness Nurse Near Canton, MI

Wellness Nurses improve the quality of our residents’ lives by helping them live in a healthy and safe manner. Through assessments, in-services, medication management and daily rounds, our Wellness Nurses work with our Resident Care Managers to ensure that our residents’ are leading healthy and happy lives. Our Wellness Nurses are a part of the Resident Care team and report to the Executive Director. They value the satisfaction that comes from enhancing the quality of life of our residents and the relationships they establish with them.

Our Resident Care Department provides individualized care to seniors who are in need of assistance with personal tasks. Our care associates assist residents with their everyday needs such as bathing, toileting, dressing, mobility, medication administration, and engagement in social activities. Through constant compassion, patience, and understanding, they strive to maintain resident independence and dignity.

Main Job Tasks and Responsibilities:

  • Oversee and ensure quality assurance by monitoring completion and quality of assessments, referrals, coordination of services and care plans for residents.
  • Provide wellness care for our residents, and follow up with resident concerns or health issues.
  • Administer and assist with medication management in order to assure that all residents are receiving the correct medication at the proper time.
  • Assess vitals of residents and report any changes to the doctor on file in order to assure that the residents’ are maintaining a high level of health. This includes completing assessments and assisting in managing service care plans for residents.
  • Assist with doctor days, care conferences, and care plan creation.
  • Create an educational environment by training associates on various physical and behavioral health topics. Attend in-service training to continue providing excellent care to our residents.
  • Maintain all regulatory compliance for our licensed assisted living and memory care communities.
  • Assist with medication management and maintaining accurate physician and pharmacy orders for residents.
  • Be a part of the on-call rotation within the department.
  • Assist with all other duties as assigned by the Executive Director.

  • Integrity and Trust – Does not take the easy road, and does not misrepresent self for personal gain. Always does the right thing, even when it does not benefit them directly.

Job Requirements:

  • A licensed Practical Nurse license is required in the state in which you are applying.
  • CPR certification is required.
  • 1 year previous experience as a Nurse is preferred.
  • Previous experience working in an assisted living or memory care community is preferred.

Physical Requirements:

  • Constant use of manual dexterity, auditory and visual skills and gross motor skills with frequent use of bi-manual dexterity and fine motor skills.
  • Ability to follow written and oral instructions and procedures.
  • Associates in this role are occasionally exposed to hot water and cleaning chemicals.
  • Ability to reach, bend, twist, squat, kneel, push and pull.
  • Ability to lift/carry up to 50 pounds. 80 pounds of pressure may be placed on the associate through transfers using transfer belts or mechanical assists.
  • Ability to perform repetitive movements including simple grasping, pushing, pulling and fine manipulation.
  • Constant standing and walking.
