What You’ll Need: 

1. Patient/Family Centered Care: Utilize the nursing process to deliver compassionate care, placing patients and their families at the center of every decision. 2. Patient Discharge & Referrals:Participates in multidisciplinary discharge planning referrals to hospital and community resources as needed.
