The Advanced Practice Provider will plan, implement, and evaluate care based on the supervisory agreement with the collaborating physician. The APP will consult with the patient and members of the health care team to provide for acute and ongoing health care and referral of the patient. Manages the prescribed medical plan of care, based on best-practice protocols and guidelines adopted by the APP and the collaborating physician. Initiates and maintains accurate records, appropriate legal documents and other health and clinic reports. Participates with the physician in periodic review of the patient's health records.

The APP provides services in accordance with the policies of the medical facility. Arranges for or refers patients to needed services that cannot be provided at the clinic and assures adequate health records are maintained and transferred as required when making a referral. Participates in periodic and joint evaluation of services including chart reviews, patient evaluations, and case outcome statistics. Participates in the review and revision of adopted protocol for medical care plans. Assists in the implementation of required quality programs.

Requirements:

• Licensed by the State of Illinois as a Certified Physician Assistant or Advanced Practice Registered Nurse.
• Graduate of an accredited university.
• Current CPR training / ACLS preferred.
• Certified First Assist Preferred.
• Clinical experience in orthopedic clinical practice.
• Strong communication skills.
