We are an approved behavioral health home organization providing integrated, whole-person, culturally sensitive care. Our team is dedicated, dynamic and experienced. We are looking for an individual with an MHRTC or Provisional MHRTC to work directly with a targeted community. We are a program that promotes professionals to provide excellent care while in a supportive, trauma-informed environment. Reports To: Program Manager/Clinical Director
Position Overview: The Case Manager/Health Home Coordinator for Members with Serious and Persistent Mental Illness (SPMI) will draft the Plan of Care for each client, oversee that Plan of Care and the coordination of services, and support and encourage client to actively participate in reaching the goals set forth in their Plan of Care.
