Wabanaki Case Management a division of Cornerstone Behavioral Healthcare is Hiring a Community Integration Specialist (Case Manager) Near Bangor, ME

Are you someone who is passionate about helping others, who experience mental illness, achieve their goals? If so, we are looking to hire case managers for Wabanaki Division of Cornerstone. We are an approved behavioral health home organization providing integrated, whole-person, culturally sensitive care. Our team is dedicated, dynamic and experienced. We are looking for an individual with an MHRTC or Provisional MHRTC to work directly with a targeted community. We are a program that promotes professionals to provide excellent care while in a supportive, trauma-informed environment. We are a small program, with many benefits to offer and have competitive rates. We have a low turnover with high staff satisfaction.

Reports To: Program Manager/Clinical Director

Position Overview: The Case Manager/Health Home Coordinator for Members with Serious and Persistent Mental Illness (SPMI) will draft the Plan of Care for each client, oversee that Plan of Care and the coordination of services, and support and encourage client to actively participate in reaching the goals set forth in their Plan of Care.

Position-specific Duties

  • Welcome, identify, engage, and serve individuals with co-occurring substance use and mental health disorders, and to incorporate attention to these issues into program content
  • Coordinate or conduct the completion of a comprehensive bio-psychosocial assessment
  • Develop the Plan of Care; honoring voice and choice of the client; oversee and the coordination of services of said Plan of Care, and encourage members in actively participating in reaching the goals set forth in their Plan of Care
  • Work with PCP to integrate behavioral and physical health care
  • Develop client centered safety/crisis plans and participate in crisis resolution
  • Monitor and evaluate quality of services
  • Coordinate and participate in team meetings with client, guardian, and other service providers and natural supports
  • Refer and link client to community-based resources and supports
  • Transport/Accompany client into community
  • Advocate for client rights and access to resources for unmet needs
  • Assist, monitor, and support client in transitioning between levels of care
  • Meet with clients on a regular basis and as is clinically necessary at minimum 1x/1 hour per month face to face
  • Develop relationships with representatives from other agencies to support clients
  • Maintain professional integrity, ethics, and boundaries
  • Perform other duties as related to the community integration specialist position

Administrative Duties

  • Submit documentation in the electronic health records system
  • Maintain client records and progress notes in compliance with regulatory requirements as developed by DHHS, MaineCare, and Cornerstone Behavioral Healthcare
  • Enter authorization requests in KEPRO

Requirements

Must:

  • Have Mental Health Rehabilitation Technician/Community (MHRT/C) or Provisional Certification
  • Have 1-2 years of relevant experience working with persons with mental illness preferred, but willing to train
  • Become trained in LOCUS
  • Train in the needs of AMHI consent decree clients
  • Work within a multidisciplinary team- Psychiatric Consultant, Medical Consultant, Clinical Team Lead, Nurse Care Manager, Certified Intentional Peer Support Specialist
  • Maintain Certifications and CEU requirements
  • Maintain a valid driver’s license and proof of vehicle insurance (100,000/300,000)
  • Be able to remain in a stationary position fifty percent (50 %) of scheduled day
  • Be able to move and transport up to 25 pounds
  • Be able to visit clients in their home and community where ascending/descending stairs may be required
  • Be able to move about inside the office, clients’ homes, and community with possible exposure to second-hand smoke
  • Be able to operate basic office equipment, e.g., computer, telephone, copy/fax machine, printer, etc.
  • Be detail-oriented
  • Be able to maintain a high level of confidentiality
  • Be able to learn new skills and grow as the position entails
  • Have the ability to communicate or exchange information in an easy-to-understand manner
  • Have the ability to comprehend basic instructions and be oriented to the position
  • Have the ability to deal effectively with the public as well as co-workers on all levels
  • Possess good organizational and time management skills
  • Possess good problem-solving skills and initiative

Job Type: Full-time

Pay: From $22.00 per hour

Expected hours: 40 per week
