Voyager Home Healthcare (https://voyagerhomehealthcare.com/) provides in-home Nursing, Certified Nursing Assistants, Physical Therapists, Occupational Therapists, and Personal Care Worker services throughout the state of Colorado (Denver, Pueblo and Colorado Springs). Additionally, we have two CNA schools (https://www.getcna.co/) which are unique in that they allow students to take their classes in an online format while enabling them to attend in-person clinical and practice labs. Program Overview:

The Parent CNA program (Family Member as a CNA Program) is a state-run program that allows parents to obtain their CNA certification and provide CNA care to their children. We are currently looking for a Registered Nurse (RN) to case manage our Parent CNA pediatric patients. As the RN Case Manager, you will be responsible for:

At Home Work

The case management side of this position can be done fully from the comfort of your own home. Voyager will provide a MacBook Pro for hired candidates to use. There are in-home visits nurses complete that consist solely of assessments. These assessments are PATs, Non-Oasis Start of Cares, and Recertifications.

Clinical Tasks

• Non-OASIS Start of Care
• Non-OASIS Recertifications
• Non-OASIS Discharges
• Change of condition visits
• Pediatric Home Health assessment Tool
• Supervision of the CNA
• Nursing interventions as needed

Administration

• Obtaining physician orders
• Care coordination
• CNA skills verification

Hours

We are looking for an RN Case Manager to work 12 hour shifts (7am-7pm).

The RN has the option for a schedule of Friday-Sunday or Saturday-Monday.

Training

Training for this position takes place in Colorado Springs and can last between 5-12 weeks. The training schedule will mirror one of our current RN Case Managers, most likely Sunday-Wednesday and/or Wednesday-Friday. The training schedule will be made 2 weeks in advance, every 2 weeks.

Job Type: Full-time

Pay: $40.00 - $45.00 per hour

Expected hours: 36 per week
