Volunteers of America SELA is Hiring a Health Navigator Near Orleans, LA
JOB SUMMARY
This position is responsible for providing quality health navigation and medical care coordination services to women who are African American females living with HIV in our service area. This position will ensure that quality care outcomes are achieved through navigating the individual through community and outpatient medical options and referral resources. This position will also be part of a team who will conduct the evidence-based interventions to the program participants in a manner prescribed by the grant evaluators and the fidelity sheets of the interventions.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Develops and implements health navigation individualized service plans based on acuity to ensure consumers are making progress towards their goals and objectives.
- Complete individual educational sessions with clients during the 12-month time frame of program.
- Completes documentation as required by VOASELA and regulatory agencies/funding sources.
- Serves as advocate for individuals receiving services.
- Prepares accurate and timely documentation for reporting purposes and follow-up.
- Keeps supervisory staff apprised of patient navigation and evidence-based intervention efforts to program participants.
- Participates in program related training, to include any HIPAA compliant telehealth trainings, as required.
- Providing mental health/counseling interactions relating to the program.
- Work with the Data Manager, local and national evaluators to maintain quality control efforts and gather necessary documentation and data collection. This position will follow all research processes and protocols and collect data accurately and in a timely fashion.
- Facilitates meetings or interactions focusing on empowerment, education, self-advocacy and self-maintenance.
- Performs related duties as assigned.
KNOWLEDGE, SKILLS AND ABILITIES
This position requires the incumbent to have completed education at the Bachelor s level preferred, with mental health knowledge and background working in this field; one year care coordination minimum and experience with working in a community setting; experience with Trauma Informed Care a plus, have a demonstrated willingness to be responsive to the needs of the target population, excellent verbal and written communication skills and the ability to function as part of a team; can handle a flexible work environment; have strong decision making skills. Some driving within the Northshore and Southshore regions will be required to attend to client needs.
