Volunteers of America Ohio & Indiana is Hiring a Community Health Nurse Supervisor Near Indianapolis, IN

SUMMARY: 

The Community Health Nurse improves health outcomes by providing both clinical support and health education to staff, clients and community partners. They facilitate communication between people, their families, and the medical community in order to improve health outcomes. This role is instrumental in program development, clinical care, and coordination of services to improve health outcomes.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Engage community partners (health systems, FQHC, PCP offices etc) to educate them on our programs. 
  • Train new community partners on the 4P’s Plus SUD assessment and the process to refer positive screenings to VOA. 
  • Pre-natal coordination including referral to OB, Mid-wife, or doula.
  • Pre-natal education for clients
  • Pre-natal supply coordination for clients
  • Postpartum or loss follow ups with clients weekly for 12 weeks after delivery or demise. 
  • Perform and record vitals during weekly visits. 
  • Referrals and coordination for lactation services.
  • Follow up and coordination in relation to child immunization.
  • Documentation of each client contact. 
  • Supervise a team of RN Community Health Nurses.
  • Assist Behavioral Health management in the creation and development of programs. 
  • Report outcomes and statistics as needed to Agency leaders..

Clients successfully complete the treatment program and attain their goals; the program maintains standards and certification; the program and Volunteers of America have a positive public and professional image.

SUPERVISORY RESPONSIBILITIES:

A team of Community Health Nurses 

QUALIFICATIONS and COMPETENCIES: 

Education Experience & Licensure

  • Must be a registered nurse in good standing with the Indiana Professional Licensing agency (IPLA).
  • A valid Driver’s License is required and active car insurance. 
  • Must have certification/willingness to obtain certification in first aid and Adult and Child CPR.A Nursing license (RN)

  • Must have excellent written and verbal communication skills and and be able to communicate appropriately and effectively in sensitive and/or demanding situations.
  • Must demonstrate the ability to professionally represent Volunteers of America, and effectively interact with multi-cultural and diverse populations.
  • Ability to operate in a trauma-informed manner with clients and staff. 
  • Ensure that client environment is one that is therapeutic and promotes healthy recovery from addiction.
  • Ability to maintain a high level of cultural competence in order to be effective with clients and operate successfully within a diverse team. 
  • Proficiency in issues related to addiction and recovery. 
  • Ability to work independently and have a high level of integer.
  • Ability to provide accurate and objective documentation within an Electronic Health Record (EHR). Ability to work with several computer programs including but not limited to: Microsoft Excel; Outlook; Word; PowerPoint and Publisher.

Physical Demands: While performing the duties of this job, the employee is regularly required to talk and hear. The employee frequently is required to sit, stand, walk, use hands requiring manual (finger) dexterity, reach with arms and hands; occasionally required to climb or balance, stoop, kneel, crouch, crawl. The employee may from time to time be required to lift and/or move up to 25 pounds. Specific vision abilities required include close vision, distance vision, depth perception, and the ability to adjust focus.

Work Environment: May interact withresidents in a variety of supportive or residential housing including a transitional dormitory for the homeless, alcohol and drug addicted and chronically mentally ill. Environment is periodically hectic and as a result may become noisy. Incumbent may be required to intervene in potentially volatile situations. Position requires work schedule flexibility to include evening hours and possible weekend hours and travel throughout Ohio and/or Indiana. While performing the duties of this job, the employee may be exposed to weather conditions. The noise level in the work environment is usually moderate; the pace and demand for multitasking, can become hectic at times. Position may require work schedule flexibility and travel.
