About the job:
The Registered Dietician screens and assesses participant nutritional status for Senior Community Care participants. Uses pertinent data to plan and implement appropriate nutrition interventions and communicates the information to the Interdisciplinary Team to ensure the nutritional needs of the participants are met. Performs nutritional assessment and provides nutritional education for Senior Community Care participants requiring interventions. Coordinates serving of meals; monitors/coordinates kitchen facilities to ensure standards are met. Coordinates with contracted meal services to ensure meals meet the needs of the Senior Community Care participants. Provides education to other health care professionals as appropriate. Provides direction and supervision to Dietary Aide.

Senior Community Care of Colorado (SCCCO) is a PACE Program (Program of All-Inclusive Care of the Elderly) serving older adults in Montrose and Delta Counties. Our team offers wrap-around care services to meet the unique needs of each of our participants and support them to remain living independently in their homes and communities.
