Description
**The Intake Specialist assists human service programs in identifying eligible veterans and resources in rural
communities to meet the needs of persons seeking assistance in the Supportive Services for Veteran Families
(SSVF) program. Conducts initial assessment functions, screening for eligibility, conducting intake and making
appropriate referrals for callers and walk-ins. Identifies and educates rural organizations in order to
coordinate services. Engages with rural housing initiatives and private Landlords to educate them on the SSVF
program and assist veterans in obtaining housing in competitive markets. Assists with front desk staffing as
needed. Job duties include supporting and coordinating with All CO case managers to ensure emergency and
urgent needs of veterans are met in a timely manner. Provides follow-up to clients as assigned and maintains
positive interactions with community stakeholders.
ESSENTIAL DUTIES AND RESPONSIBILITIES
· Receives and assesses calls for service and conducts intake process: screening households for eligibility,
collecting relevant documentation, and supporting SSVF program staff throughout the enrollment process
at the VSC and in the community.
· Ensure eligible Veteran client’s case files are entered in case management database for approval per
supervisor. Refers ineligible applicants to other organizations/programs which would best serve the
applicant’s needs. Provide and document appropriate referrals for those screened but not eligible.
· Maintains and updates a database of active community supports for the purpose of linking low income
and/or homeless veteran households with available community resources. Contacts and maintains
dynamic relationships with these organizations to ensure that they remain aware of our assistance.
· Assists with outreaching landlords and hotels to work with the SSVF program. Providing education about
our services. As well as assisting with inspections of potential units for veterans.
· Utilizes the models of Trauma-Informed Care and Housing First to guide engagement with existing and/or
potential SSVF program participants.
· Provides administrative and programmatic support services for client service teams.
· Documents all eligibility screening activities through paper and electronic logs.
· Identifies potential housing for Veterans living in a 17-county territory. Works with Case managers and
Landlords to explain program and buy in.
· Helps to identify community resources in rural and underserved areas to ensure needs are met- this can
include short term emergency housing assistance for homeless veterans who are awaiting housing
placement.
· Maintain/track application/screening data inflow and report outcomes.
· Attends staff, team and department meetings.
· Supports SSVF staff in getting paperwork signed with clients including certifications, recertifications, grant
per diem applications and releases of information.
Travel to 17 county service area is required on a regular basis and may include up to 50% travel within the work week.

Salary: $21.00-$23.00/hr
POSITION TYPE AND EXPECTED HOURS OF WORK
· Full-Time or Part-Time as indicated in posting.
· Works hours vary depending upon program requirements but are typically scheduled around a 40-hour work week.

MINIMUM QUALIFICATIONS OF POSITION
· Associate degree in communication, human services, administration, or related fields, Or 1-year direct services experience in homelessness, veteran services or other related fields
· Must complete agency and program credentialing within 12 weeks of hire and maintain credentialing standards thereafter.
· Proficient computer skills, to include proficiency with the following: Microsoft Office Suite (Word, Excel, PowerPoint, Outlook); communication and collaboration tools (Zoom, Teams); digital calendars (Google, Outlook); file sharing programs (Dropbox, Google Drive); and project management software (Asana)
· Proficient with Windows operating system.
PREFERRED QUALIFICATIONS OF POSITION
· 1 year of related experience.
· Demonstrated ability to de-escalate clients in crises.
