Telehealth nurse summary:

Identify High Risk for Hospitalization patients by utilizing Analytics Report (High Risk SOC, High Risk care type discharge). Contact High Risk Patients and document vital signs, high risk red flags, and initiate action plan: MD notification, SN visit, and PRN orders. Provide clinical suggestions to supervisor and field staff regarding High Risk Care such as: Telehealth Calls, Discipline Referral, High Risk Issues. Coordinate Telehealth Calls for High Risk Patients: weekly until discharge, monthly following discharge. Review Admission Visit, Plan of Care, and Disease Management Program needs. Communicates with Clinical Managers on identified patient outcomes via Touch Program. Works with Disease Management Programs Developer.
