Job Description:
The Triage Registered Nurse is responsible for triaging calls that come in during weekend hours and providing skilled nursing visits to patients in accordance with Medicare conditions of participation, Title 22, nursing standards of care, VNA Health’s policies and procedures and accrediting bodies. The Evening Triage Registered Nurse promotes the mission, values and vision of VNA Health.
Responsibilities:
  • Makes PRN home care visits for the Hospice Departments between 5:00 p.m. and midnight, Monday-Friday and 4:00 p.m. and midnight Saturday and Sunday. This includes home visits at the time of death for hospice patients and visits when evening RN has no availability.
  • Carries a cell phone and answers incoming calls to the agency during the evening shift.
  • Triages phone calls forwarded by the telephone exchange.
  • Performs assigned visits &/or telephone calls.
  • Ensures physician orders (plan of care) are followed as written.
  • Reports changes to physicians and case managers as evidenced by documentation.
  • Completes all documentation requirements of the agency by the end of the shift.
  • Participates in educational programs to maintain skills and knowledge of current nursing practices.
  • Attends Hospice department staff meetings.
  • Supports agency Performance Improvement program.
  • Performs other related duties as assigned.

All clinical staff are expected to participate as needed in variable shifts, including weekend rotation per staffing needs and departmental holiday rotation.
Qualifications:
  • Current and active California RN license.
  • Bachelor’s degree, preferred.
  • Minimum one year acute care experience required.
  • One year recent hospice experience preferred.
  • Current CPR certification.
  • Licensed driver with insured automobile - in accordance with state and/or organization requirements.
  • IV certification required prior to completion of six month introductory status.
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
  • Ability to respond independently to life threatening situations.
  • Ability to reason, deduce and carry out complex multiple step processes.
  • Bilingual – Spanish preferred, but not necessary.
Physical Requirements:
  • Ability to lift 25 lbs.
  • Ability to push/pull 50 lbs.
  • Ability to stoop, bend, and squat.
  • Ability to walk up one flight of stairs.
  • Ability to walk one mile.
  • Ability to sit for one hour, minimum.
  • Visual and audio acuity.
