POSITION DESCRIPTIONPOSITION TITLE: Certified Nursing AssistantREPORTS TO: Director of NursingDEPARTMENT: NursingThe primary responsibility of your job position is to provide each of your assigned residents with routine daily nursing care and services following the current federal, state and local standards, guidelines and regulations that govern long term care and assisted living facilities. You are entrusted to ensure that the highest degree of quality care can be provided to our residents at all times.KEY RESPONSIBILITIES:Receive and report resident status at the beginning and end of each shift and as needed Provide personal care to residents per their individualized plan of careDocument all pertinent information regarding care as assignedReport all changes in residents’ conditions to the Nursing Supervisor as soon as practicalPrepare residents for meals and assist to and from the dining areasRecord residents’ fluid/food intakeAssist residents with toilet, dental, hair, and bath care Keep residents dry and cleanMake room checks and verify resident locationKeep resident’s rooms neat and cleanAssist residents with dressing/undressing as necessaryPrepare residents for activity and social events (church, services, parties, visitors, etc.)Transport residents to/from appointments, activity and social programs, etcAssist residents to walk with or without self-help devices as instructedPerform vital signs, B/Ps, weights and other trained procedures as directed by management Complete prevention and restorative care that includes but not limited to PROM, AROM, ambulation, transferring, bathing, grooming, and all other ADL careAssist in turning, positioning, and repositioning of residentsAttend care plan meetings as assignedDemonstrate a passion for caring as evidenced by interaction with co-workers, residents, families, and visitorsMaintain and support a culture of compliance, ethics and integrityMaintain confidentiality and rights of resident, family, employee, and facility information in compliance with PHI HIPAA policies and applicable regulationsMaintain safe work environment and exhibits safe work practices Perform all other duties as assignedTALENT:Relationship BuildingHave a caring attitude and sincere interest in othersDemonstrate friendliness and enthusiasmBuild relationships with all types of people InfluencingCommunicate effectively ExecutingSelf-directedAbility to organize and prioritizeAbility to exercise judgment and display understandingDetail oriented and follow through Strategic ThinkingSolve problems effectivelyCreative in coming up with options and varietyCuriosity and desire to learn SKILLS AND KNOWLEDGE:Google and Microsoft operating system platformsAdministrative and clerical procedures; customer serviceEDUCATION AND EXPERIENCE:Must possess, as a minimum, a 10th grade educationGraduate of a state approved nursing assistant training program and passed a state approved competency examination (written and skills) within 120 days of employment name entered into state nurse aide registryJOB TRAINING: How long it usually takes to completely perform this job when the incumbent has the minimum knowledge, skills, ability, work experience and/or education: up to 6 monthsWORK ENVIRONMENT:Works in all areas of the facilityMoves intermittently during working hoursIs subject to frequent interruptions Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstancesIs subject to hostile and emotionally upset residents, family members, etc. Communicates with facility staff and department supervisorsMay work beyond normal duty hours, on weekends, and in other positions temporarily, when necessaryIs subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.)Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminants Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B virusesMay be subject to the handling of and exposure to hazardous chemicalsPHYSICAL AND SENSORY REQUIREMENTS (with or without aide of medical devices)Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facilityMust meet the general health requirements set forth by the policies of this facility, which may include a mental and physical examinationThe physical and sensory requirements described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
