Responsibilities:

• Provides skilled nursing services as per care plan.
• Documents patient observations, interventions, and goals.
• Delivers and documents patient care during visits.
• Communicates changes in patient condition to the clinical team.
• Submits documentation as per agency requirements.
• Meets productivity standards and clinical competencies.
• Uses agency-provided telecommunication devices appropriately.
• Follows standards for technology and documentation.
• Engages in education, quality improvement, and protocol development.

Qualifications:

• Licensed Vocational Nurse License in California
• Current CPR/ BLS Certification
• Physical and TB tests (within one year)
• Driver's License or State ID
• IV Certification
• Home Health experience is a plus
• Ability to drive within 30-60 miles a day
• Reliable transportation and insurance

Pay: $45.00 - $50.00 per hour

Expected hours: 40 per week
