Vitality Hospice & Palliative Care is seeking an exceptional Licensed Practical Nurse (LPN) to join our team as a Clinical Team Coordinator! *Must have the required LPN to be considered for this Full Time position, on-site in our Wall, NJ office*

Responsibilities:

The Clinical Team Coordinator (LPN) is responsible for ensuring that hospice care and services are coordinated among all Hospice team members appropriately, and according to the Hospice Care Plan. The Clinical Team Coordinator (LPN):

• In conjunction with the Intake Coordinator and Director of Clinical Operations will review incoming referrals, discuss hospice services with families/referral sources and assist with the admission process.
• Is responsible for the day-to-day clinical coordination with the Hospice team, including processing admissions, recertifications, changes in level of care, live discharges, and changes in condition to ensure patient needs are met.
• Tracks and monitors all Infections and Incidents on the Infection and Incident Logs, ensuring that the corresponding reports are completed by the appropriate Hospice team member.
• Oversees DME needs, medication needs, supplies for wound care, pharmacy information in conjunction with the RN Case Manager.
• Calls and communicates with the referring facility or doctor to verify/receive orders.
• Educate patients and caregivers as appropriate.
• Will be available and willing to perform visits to patients places of residence for routine visits, PRN visits, as needed.

Qualifications:

• Current Professional Nurse license
• Must have a valid drivers’ license with reliable transportation and liability insurance
• Ability to read and follow written instructions and document care given.
• Self-directing with the ability to work with little direct supervision.
• Empathy for the needs of the ill, injured, frail and the impaired.
• Strong communication and interpersonal skills
• Good time management/calendaring skills and ability to deliver documentation timely

Job Type: Full-time

Pay: $25.00 - $30.00 per hour
