Currently, ViTalent is working with the largest healthcare facility in Socorro, NM to find Home Health Registered Nurses for 13 week contracts!
Great pay at $57/hr ($2,280/wk) with weekly direct deposit!
HH Case Manager RN Contract
- 13 weeks with extension options
- 40 Guaranteed hours per week
- Day shifts
- 8 hour shifts (8a-4:30p)
- 1 wknd rotation/month
- No on-call
Experience: 1 year
Charting System : EPIC preferred
- patients requiring Home Health, Infusion and Pediatric nursing, and/or Palliative and Hospice services.
- Conducts initial assessment
- Establishes the plan of care, Provides direct patient care, Provides on-going assessment of patient response to treatments and teaching and applies interventions as appropriate. Updates and revises plan of care as indicated.
- Completes documentation including visit assessment, OASIS, 485 and verbal, Communicates with physicians to confirm and update patients' plan of care, Facilitates the discharge planning process according to standards and utilizes resources to assure continuity of care after discharge.
