Responsibilities include (but are not limited to):

• Review nursing assessment reports
• Communicate with visiting nurses and coordinators to verify information and resolve issues.
• Ensure DOH and HHA Exchange requirements and standards are met.

Requirements:

• Minimum of three (3) years of experience in home health care as visiting nurse
• Self-motivated and reliable work ethics
• Detail-oriented, well organized, and efficient work approach
• NYS licensed and currently registered as a Registered Professional Nurse
• Experience and knowledge with HHAeXchange and nursing assessments
• Graduate of an accredited school of nursing with a diploma, baccalaureate, or master’s degree
• Working knowledge of NYSDOH rules and regulations
• Strong problem solving, organizational, written, communication, and interpersonal skills.
• Verifiable personal and professional references required.
• Proficiency in basic computer skills, Microsoft Outlook, Adobe PDF, etc.
