Vista Community Clinic is a private, non-profit, multi-specialty outpatient clinic providing care in a comprehensive, high quality setting. Responsibilities

Provide assistance to LGBTQ community members in housing navigation and referrals to wraparound services including medical, social, job readiness, and more. Provide assessment, education, and referrals to individuals and households to appropriate services. Establish and help maintain communication between patients, partner organizations, clinicians, and the health care system. Work to eliminate barriers to care for program participants. Assist in program activities and administrative support. Collaborate with the HIV Prevention team on outreach and testing events.

• Provide education, navigation services, and referrals to improve health care outcomes and reduce health disparities in individuals who identify as LGBTQ who are unhoused or at risk of being unhoused

• Provide education, navigation services, and referrals to improve health care outcomes and reduce health disparities in individuals who identify as LGBTQ who are unhoused or at risk of being unhoused
• Complete individualized sessions with clients to perform assessments, complete housing plans, track progress on client goals, and gather feedback
• Conduct outreach and provide HIV health education, counseling and testing services to individuals at risk for HIV infection or transmission

Qualifications

Minimum

• One year experience working in community outreach, health education, and/or housing services
• High school diploma or equivalent 
• Valid California driver’s license, current vehicle insurance and reliable transportation
• Bilingual English/Spanish
• Ability and willingness to work flexible hours, including evenings and weekends

Required Skills/Knowledge/Abilities

• Knowledge of HIV-related social and emotional issues, demonstrated competency in working with culturally diverse, low income or no income clients
• Preferred skills include demonstrated competency working with LGBTQ, persons in recovery, homeless individuals, and other marginalized populations
• Ability to present information accurately and effectively both verbally and in writing to individuals and groups
• Good verbal and written skills necessary for communication with patients/clients, providers and other staff
• Experience/familiarity with computers, Microsoft Office products (Word/Excel), business e-mail communication systems and internet search capabilities

Pay Range

$18.00 - $21.00
