Responsibilities:
  • Creates appropriate and thorough documentation of any appointments and ensures any medications are accounted for in all systems, including, but not limited to: health tracking, GERS/incident reports, T-Logs, personal hygiene, vitals and time tracking, overnight call circle, secure communications (Scomms), behavior data, safety data, fall data, seizure data, and the daily activity calendar. 
  • Models and promotes the delivery of quality services through direct care duties and in partnership with DSPs 
  • Provides professional and routine communication with staff members, guardians and funders and other third-parties 
  • Notify leadership immediately of any health and/or safety concerns of the individuals supported. 
  • Address personal shopping needs for individuals supported 
  • Maintains accurate financial records and reporting for all purchases for the home as for individuals supported, following operations and accounting policies, procedures and practices. 
  • Responsible for monitoring and providing for the appropriate supplies, groceries and household items to meet the needs of individuals supported. 
  • Assist individuals supported with different tasks as necessary, including meal preparation, bathing, clothing and toileting as necessary based on that individual’s needs and any behavioral support plans 
  • Performs toileting, including providing and addressing bedpans, urinals and commode chairs as necessary; ensures individuals who are incontinent are changed immediately using appropriate care, and observe for any changes of condition; report any issues immediately to Vista Care leadership 
  • In collaboration with the Service and Support Director (SSD), ensures that all residential facilities under their purview, have the appropriate staffing coverage and fills in any gaps as necessary to ensure proper ratios and the highest quality of care; including the need to cover any programs across the region, or as assigned 
  • Responsible for responding to program needs that may fall outside of assigned hours; works with SSD to adjust/flex hours to accommodate and adapt to changes and needs of the programs.
  • Promotes and models VC mission, values and vision, expecting the same from DSPs 
  • In coordination with the SSD, ensures that all programs meet all regulatory standards and that staff act in accordance with Vista Care’s values, policies and procedures 
  • Conducts monthly program inspections and audits 
  • Provides frequent communication to all staff regarding any changes impacting the individuals supported, company communications and changes and other relevant information obtained through leadership, emails and Scomm’s 
  • Provides ongoing leadership, mentorship, training and direction to all staff and in the assigned programs 
  • Acts as a change agent for Vista Care as the company continues to grow and support more individuals; is adaptable and flexible
Qualifications:
  • At least 18 years of age.
  • High school diploma or equivalent (higher education in a related field is a plus).
  • Clean driving record is required.
  • Previous experience in caregiving, healthcare, or related roles. 1 year is required.
  • Knowledge of mental and intellectual disabilities and a willingness to learn.
  • Strong interpersonal skills and the ability to build rapport with individuals and their families.
  • Patience, empathy, and a compassionate approach to caregiving.
  • Ability to follow directions, work independently, and make responsible decisions.
  • Excellent communication skills, both verbal and written.
  • Flexibility in scheduling, including availability for evenings, weekends, and holidays.
  • First Aid and CPR certification (or willingness to obtain upon hire)
