In this role, you'll care for patients in Kosciusko, Whitley, Noble, DeKalb, Huntington, & Northern Allen counties.

You will be working40 hours each week.

POSITION OBJECTIVE: 

Manages and Coordinates provisions of nursing care for assigned terminally ill palliative care patients by assessing needs, implementing nursing
care plans, making appropriate revisions, and evaluating outcomes of care.QUALIFICATIONS: 
  • Graduate of an accredited school of nursing. Bachelor's degree in Nursing preferred.
  • Must have a current RN license to practice in the state of Indiana.
  • Must hold current CPR certification or obtain it within the orientation period.
  • Infusion skills preferred.
  • Must be able to provide those services requiring substantial specialized nursing skills.
  • Prefer one (1) year of recent hospital or home health care experience.
  • Must have a valid driver's license and have access to an automobile with current automobile insurance coverage.
  • Must be able to drive independently to all patients'/family clients' homes.
  • Must be able to safely perform the job's essential functions without posing a direct threat to the health and/or safety of him/her or to others in the workplace.
  • Must be comfortable working with hospice patients, emphasizing palliative care.
  •  Assists with in-servicing the community on the hospice philosophy as needed/requested. 
  • Ability to recognize the needs and emergencies of hospice and palliative care patients. and provide appropriate interventions.
  • Strong interpersonal skills.
  • Strong assessment skills. The published compensation range for this position reflects a good faith estimate of starting base pay for full-time (40 hours per week) employment at the time of posting. The base pay may be higher or lower based on demonstrated job-related experience.
