We are looking for dedicated and skilled Registered Nurses (RNs) to join our dynamic home health team. Our commitment to excellence in patient care has earned us a reputation as a leader in the field of home health services. Join us in making a difference in the lives of our patients every day.

Oversee primary caseload of Home Health patients and provides skilled professional nursing care to them as prescribed by the physician and in compliance with the Home Health Medicare Conditions of Participation, PA licensure laws and agency policies and procedures. Provide the comprehensive nursing assessment of patient needs and collaborates to develop the patient plan of care. Implement the plan of care and reevaluates and revises the plan of care relative to the patient’s nursing needs to provide appropriate, comprehensive and responsive goals and interventions. Work closely with other members of the team to ensure all patient care needs are being met.

Primary Duties:

Assessing and Planning

  • Conduct comprehensive assessment of patient in a thorough, accurate, compassionate and timely fashion.
  • Utilize case management techniques and the nursing body of knowledge to formulate and individualize the patient plan of care to include problem identification, interventions and goals.
  • Contribute to identifying both active and potential needs of the patient which may affect the plan of care.
  • Consistently collaborate with the physician to plan and implement the medical and nursing plan of care.
  • Facilitate and participate in case conferences and contribute to patient care planning.

Implementing and Evaluating

  • Provide nursing interventions and appropriate preventative and therapeutic nursing procedures.
  • Collaborate with team members to provide continuous assessment of assigned patients, interpret findings, initiate appropriate therapeutic actions, and evaluate the results of these actions.
  • Accept responsibility of assignment to perform a specialized procedure only when qualified with specialty training, proven competency, clinical background, and expertise in the specialty area.
  • Work in cooperation with family/caregiver and team members to meet the physical and emotional needs of the patient.
  • Provide appropriate information and explanation to the patient and patient’s family.
  • Facilitate patient and family education, to promote continuity of care and optimal patient outcomes.
  • Demonstrate expertise in the utilization of agency services and community resources.
  • Fulfill productivity requirements on an average daily census basis as determined by acuity level and Clinical Manager.
  • Document accurate, complete and relevant clinical notes regarding patient’s condition and care given. Maintain documentation on a timely basis.
Qualifications:
1.Graduate of an accredited registered nursing program.
2. Bachelor’s degree in Nursing preferred.
3. Has minimum of one-year general nursing experience with current home health, medical, surgical, or critical care experience preferred.
4. Ability to work within a Home Health setting.
5. Registered Nurse licensed in the Commonwealth of Pennsylvania.
6. Valid driver’s license and reliable transportation that is insured in accordance with PA state requirements.
7. CPR certified.
8. Satisfactory references from employers and/or professional peers.
9. Satisfactory criminal background check.
