Visiting Nurse Association of Central Jersey is Hiring a Ryan White Part B HIV Program Assistant Near Monmouth, NJ

Overview

Responsible for providing direct support to the Ryan White Part C Program and the LGBTQ Center for Health and Wellness at the Community Health Center of Asbury Park. Duties include provider support, patient services navigation. Role includes the management of referrals to specialists and diagnostic testing.

Responsibilities

  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
  • Coordinates medication prior authorizations for LGBTQ Center for Health and Wellness, Ryan White patients, as well as Gender Affirming Hormone Therapy (GAHT) from APCHC providers.
  • Anticipates insurance lapses, required emergency bridges and other insurance requirements.
  • Assesses for related medical program eligibility, such as community nursing services, mental health, housing services, vocational rehabilitation services, food and energy entitlements.
  • Identifies and networks with other clinical resources including medical providers, pharmacies, and laboratories.
  • Collaborates with available case management and community health workers to provide an assessment of medical, housing, mental health, legal and insurance needs developing a structured assessment and plan of care bi-annually.
  • Documents all required data into the reporting systems including CareWare. Provides reports as requested by Program Manager.
  • Adheres to agency infection control and safety policies.
  • Adheres to the organization’s policy in regards to absenteeism and appearance.
  • Tracks and collects Ryan White patient eligibility and Cap on Charges for Ryan White patients.
  • Qualifications

  •  Minimum of 2-3 years’ experience in office support role, preferably in a primary care setting
  • 1-2 years HIV/AIDS work related experience and knowledge of LGBTQ community cultural and lifestyle issues preferred
  • Knowledge of medical terminology preferred
  • Strong computer skills
  • Strong organizational and interpersonal skills
  • Bi-Lingual Spanish preferred
  • Car and valid driver’s license
