We are currently looking for a Licensed Practical Nurse to join our team. This position would be responsible for providing quality care and home health nursing care services under the direction of a Registered Nurse or an Advanced Practice Nurse.

Responsibilities:

  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  • Implements plan of care under the direction of the Registered Nurse
  • Assists client to follow preventative and rehabilitative nursing procedures
  • Monitors quality and appropriateness of services provided
  • Establishes and maintains professional working relationship with health care providers and other members of the disciplinary teams in providing patient and family care
  • Adheres to agency infection control and safety policies
  • Adheres to the organization’s policy in regards to absenteeism and appearance
  • Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position
Qualifications:
The ideal qualifications for the LPN Homecare, include:
  • Graduate of an NLN accredited LPN program with current NJ LPN licensure
  • 2 years of med/surg and acute care nursing experience
  • 1-2 years of homecare and hospice experience preferred
  • Excellent verbal and written communication skills
  • Strong computer skills
  • Excellent organizational and interpersonal skills
  • Car and valid driver’s license
  • Willingness and ability to travel as needed
Working Conditions/Physical Demand: Working indoors and outdoors with clinical equipment and use of phone and computer; Job characteristics: Client and Customer contact
