- Provides individualized counseling to patient and family regarding hospice services
- Establishes and maintains professional working relationship with hospital staff, assisted living facility staff, skilled nursing facility staff, physicians, and VNAHG staff
- Ensures compliance with regulatory and statutory requirements
- Adheres to the organization’s policy in regards to absenteeism and appearance
- Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position
- Bachelor’s degree in Nursing preferred
- Valid NJ Registered Nurse license
- 2 years off hospice nursing experience
- Excellent verbal and written communication skills
- Excellent interpersonal and organizational skills
- Strong client focus, assertive, decision making skills, articulate, and creative in problem solving
- Strong decision making skills
