Overview

The VNA of Ohio is currently seeking a full time Home Health Clinical Triage RN for West Cuyahoga territory!

As a Home Health Clinical Triage RN, you will be responsible for assessing and managing patient needs over the phone, providing expert clinical guidance, and coordinating care with interdisciplinary teams. This role requires strong clinical knowledge, exceptional communication skills, and the ability to prioritize and respond to patient needs effectively.

Responsibilities

Illustrative duties may include, but are not restricted to, one or more of the following:

  • Conducts telephone assessments of patients' health concerns and symptoms. Provide triage and clinical support to patients and their families, offering guidance on symptom management and care options. Utilizes effective interviewing techniques; sets limits, and collects data based on specific role.
  • Evaluates the urgency of patient situations and recommend appropriate interventions, including referrals to other healthcare providers or services.
  • Communicates and collaborates with physicians, nurse practitioners, and other healthcare professionals to coordinate patient care plans and secure orders for changes in medications, care plans, needed labs, etc.
  • Explains processes, procedures and treatments to patients, families, and caregivers to gain cooperation, understanding, and ease apprehension.
  • Document patient interactions, assessments, and interventions accurately and in a timely manner in electronic health records (EHR) and other software programs as indicated (i.e., Flowan).
  • Uses oral and written communication to convey pertinent information to members of thehealth care team in a timely manner.
  • Adhere to regulatory requirements, professional standards, and best practices in telehealth and home health nursing.
  • Participate in quality improvement initiatives and contribute to the development of clinical protocols and guidelines.
  • Maintain confidentiality and privacy of patient information in accordance with HIPAA regulations.
  • Complies with company policies and procedures.
  • Completes all required forms, reports, time cards, itineraries, etc, accurately and withindesignated timeframe.
  • Recognizes and follows established lines of communication.
  • Performs other duties as assigned.

Qualifications

Qualifications and Requirements:

  • Valid registered nurse (RN) license in the state of Ohio.
  • Minimum of 5 years clinical experience, preferably in home health, telehealth, or acute care settings.
  • Experience working as part of a team and working effectively with persons and communities from diverse cultural, social, and ethnic backgrounds. The ability to work in a complex institutional structure requiring significant collaboration.
  • Excellent communication and interpersonal skills, with the ability to convey complex medical information clearly and empathetically.
  • Strong critical thinking and problem-solving abilities, with the capacity to make sound clinical judgments under pressure.
  • Strong planning and organizational skills.
  • Demonstrated ability to work well under pressure, within strict time parameters, and with a strong attention to detail.
  • Ability to work independently with minimal supervision.
  • Demonstrated ability to maintain strict confidentiality.
  • Familiarity with electronic health records (EHR) and telehealth platforms.
  • Commitment to providing patient-centered care and promoting health equity.

Required Licenses, Registrations and/or Certifications:

  • Registered Nurse, licensed in the State of Ohio. BSN preferred.
  • CPR certification is required.
  • Valid driver's license.
  • Mental Health requires initial approval/certification by Medicare fiscal intermediary(Palmetto)
