Visiting Nurse Association of Central Jersey is Hiring a GUIDE Care Navigator Near Holmdel, NJ

For over 20 years, Visiting Physician Services has been bringing modern medicine into the comfort and privacy of your home.
Visiting Physician Services specializes in treating adult and geriatric homebound patients. We make house calls to private residences, senior housing buildings and assisted living facilities in Ocean, Monmouth, Middlesex, Somerset, Union, Essex, Passaic, and Bergen Counties.
Our practice is comprised of physicians, nurse practitioners and physician assistants with the mission of treating elderly, frail, homebound patients with dignity and compassion.
We are looking to hire a GUIDE Care Navigator. The person in this role will be responsible for planning, directing, implementing, and providing oversight of GUIDE program.
Responsibilities:
  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  • Plans and implements the GUIDE program to include initiatives to improve access and continuity, care management, coordination, and patient/caregiver experience
  • Develops an effective project management process to facilitate implementation and continuous improvement
  • Works closely with VP, VPS Manager, Population Health Manager, Medical Director and clinical staff to implement and monitor change initiatives, quality measures, and patient care
  • Develops tracking and reporting processes at program, patient and clinical level
  • Adheres to the CMS program guidelines and meets CMS program deliverables
  • Adheres to the organization’s policy in regards to absenteeism and appearance
Qualifications:
Job Competencies & Minimum Qualifications:
1. CMA or higher required, Bachelor’s degree and nursing license preferred
2. 5 years experience with primary care practice, housecall experience preferred
3. Strong communication, interpersonal and organizational skills
4. Car and a valid driver’s license/willingness and ability to travel
