Position Summary: Provides home health care to patients and families that focuses on preventing disease, promoting, maintaining or restoring health and minimizing the effects of illness or disability. Supervises LPN’s and HHA’s.Experience Preferred

This position is two ten hour shifts Friday and Saturday.

Territory includes: Concord, Acton, Westford, Chelmsford, Tewksbury, Dracut and surrounding towns.

• Minimum 1 year in acute care setting or comparable work experience.
