RN Case Manager, Home Health, $20K Sign on Bonus!Monday -Friday 8AM - 4:30PM, No Weekends!Overview of Registered Nurse Position:Responsible for coordinating and providing high quality multidisciplinary home health care services for patients consistent with VNA Care’s policies, goals and objectives. Works under moderate supervision.Education:Graduate of a certified RN program required; Bachelor of Science Degree in Nursing preferred.Licensure:Licensed to practice in Massachusetts.Experience:Minimum of one year of recent LTC/Rehab or med/surg experience in an acute care setting preferred; home care experience preferred.
